The Certified Nurse Practitioner performs hospice and palliative care patient evaluations in private residences, nursing facilities and assisted living facilities, provide inpatient and outpatient palliative care consults as well as liaison with attending physicians and medical directors.

Position

Qualifications:

  • 2 years of Nurse Practitioner experience in an acute care environment
  • Home Health, Palliative Care or Hospice Experience Preferred
  • Masters of Science in Nursing
  • Active unencumbered OH NP license
  • Current driver's license and auto liability coverage
  • Detailed assessment skills as part of this role require recertification for Hospice patients
  • Interest in a community-based role that requires travel between patient visits

VIEW JOBS 11/14/2018 12:00:00 AM 2019-02-12T00:00 Nurse Practitioner (Evenings or Weekends) Dayton Physicians is looking for two Nurse Practitioners for our Urology practice! This is an exempt position responsible for providing Urologic patient care under direct Physician supervision. Duties include but are not limited to assessing, diagnosing, prescribing, treating, scheduling and educating patients. This position works closely with physicians and other clinicians in a team approach to patient care in a clinical and hospital setting. Essential Duties and Responsibilities: * Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing appropriate physical examination; * Determine tentative diagnosis and order appropriate laboratory and diagnostic tests; * Consults with Physicians and other clinicians as an integral part of the clinical team to asses, evaluate, and address Urologic patient needs; * Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws; * Communicates with patient regarding medications, treatments, and discharge instructions as applicable. * Educates patients regarding health promotion/illness prevention; * Communicates appropriate case management information to members of the clinical team; * Prepares documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written/computerized manner; * Transition of care for hospital patients. Schedule patients for outpatient services when appropriate to assure proper transitional care. * Coordination of care for office patients. Field clinical phone calls and serve as contact to patients and referring Physicians; * Serves as first contact for after-hours patient and physician/heath care provider/hospital/ER calls and appropriately collaborates with our on call physician; * Follow all health care privacy and safety rules and adheres to Dayton Physicians Core Values; * Other duties as assigned. Required Education and/ Experience: * This position requires an Advanced Registered Nurse Practitioner License for the state of Ohio. * Requires prescriptive authority; * Graduate of an accredited advanced nurse practitioner curriculum; * Experience with electronic medical record systems a plus; * Current CPR certification; * Current ACLS certification; * High School diploma or GED required.
