In this role, you will represent Optum as you provide primary care to patients in long term care settings; coordinate with their Primary Care Provider and facility staff to deliver high quality care on site. You'll interact closely with the facility staff and providers throughout the community; educating, building relationships and showing leadership in how to best address the complex needs of the patient and family. This is a flexible, autonomous role that creates enormous satisfaction for the NP as you impact the care and comfort of our aging population.

Primary Responsibilities

  • Develop strong competencies in Palliative Care as you provide preventative and primary care for patients in assigned long term care facilities

  • Collaborate with primary care physicians, the nursing staff and the patients' families to coordinate care in the manner best suited to the patient's needs

Your work within these independently owned/operated long term care facilities means that you represent us not just with our members and their families but also with the employees and leaders of each facility. Balancing how to work with, educate, and service the facilities' staff while also providing the members with the clinical and caring attention can be a challenge.

Required Qualifications

  • Current, unrestricted NP license in the state of assignment, Ohio State

  • Ability to obtain DEA licensure/Prescriptive Authority post-hire in states where applicable

  • Clinical background and national certification as ANP, FNP, GNP,ACNP required or the ability to obtain

  • Past experience working in a nursing home or with seniors in other settings

  • Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook

Position Summary The HR Coordinator provides first-level Human Resources customer service and support to Metro Region operations personnel. Coordinates activities to ensure that the day–to-day operational functions of the Human Resource Department are achieved. Maintains and distributes human resources data and reports in support of decision-making and change initiatives. Coordinates the scheduling, planning and execution of employee engagement events across all Metro Region hospitals. * Coordinates and supports various Human Resource in support of our internal customers (employees and managers) which include employee relations, policy referencing and interpretation, compensation, benefits, performance management, talent acquisition and development. * Provides HR customer service to employees and managers of all levels. Possesses the knowledge and skills necessary to successfully disposition a very high percentage of inquiries that come to HR. Utilizes good judgment in escalating HR concerns and issues to the appropriate Human Resource Specialist or Leader. * Maximizes the use of Human Resource software and systems for retrieval and sharing of information. * Maintains current knowledge on compliance and regulation standards as related to employment and Human Resources matters. * Conducts or arranges for education and training for leaders and employees on Human Resource related matters. * Represents Human Resources and/or chairs councils and committees as assigned. * Coordinates all projects, meetings, and events for the Human Resource Department. * Coordinates the Human Resource office interns and summer youth workers. * Attends all meetings and in-services as required. * Performs all other duties as assigned. Job Requirements * Must have a bachelor's degree or an associate degree and equivalent experience. * Must have one year's experience in Human Resources. * Must be proficient with MS Office applications. * Must be able to type 40 wpm. * Must be able to work independently with little or no supervision. * Three year's experience in specialized function preferred. * Must be able to move, lift or carry materials weighing up to 30 lbs., 20% of the time.
