This advanced practice provider will join Community Physician Network, a multispecialty group with over 1,000 providers in Indianapolis, IN. Indiana also maintains its place as one of the top 5 provider friendly states. This provider will be located at our downtown Community Clinic at Walgreens, off of 16th and Meridian.

Highlights

Community Physician Network, part of Community Health, has an excellent opportunity for a Nurse Practitioner or Physician Assistant to work in our new Community Clinics at Walgreens. Community Health Network's Community Clinics put high quality, convenient medical care throughout all regions of Indianapolis and the surrounding suburbs.

  • Clinic hours are Monday
  • Friday 9:00 am

  • 7:00 pm; Saturday and Sunday 9:00 am

  • 4:00 pm

  • Electronic Medical Record (EPIC)

  • Autonomous practice

  • Competitive compensation and benefit package

Job Duties

Nurse practitioner or physician assistant will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine. Promotes a healthy lifestyle and involves the client in his/her plan of care with emphasis placed on patient self-determination and maximization of client knowledge to enhance care of oneself.

Experience

Minimum three to five years of family medicine or urgent care experience as an advanced practice provider is required; this is an independent position requiring a strong level of autonomy.

Education

Educational background consistent with requirements for licensure and certification with the State of Indiana

Skills

Demonstrate a high level of expert skill in delivering exceptional, quality care while enhancing the well-being of the patient

Highlights REACH stands for Resources to Evaluate and Advance Comprehensive Health. This program will require flexibility as we reach out to serve those who are marginalized in our communities. This opportunity will be focused on adult medicine specific to high-risk populations. * Clinic hours are Monday - Friday 8:00 am-5:00 pm.
