In this role, you will conduct assessments or provide primary care to patients in their homes or long term care settings. You will coordinate with their physician and facility staff to deliver high quality care. This is a flexible, autonomous role that creates enormous satisfaction for the Nurse Practitioner as you impact the care and comfort of our aging population. The Clinical Advisor, under the direction of the CTM will also assist with ride-alongs, quality assurance, and the implementation of clinical initiatives. All the while, you'll be building meaningful relationships with the patients, their families, and the health care providers who are responsible for their care.
Provide preventative, primary care, or assessments for patients in their homes or in assigned long term care facilities
Work with primary care physicians to provide the best care possible
Collaborate with the nursing staff and the patients' families
Conduct assessments on health plan members
Review patient's past medical history and formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
Review current symptoms and identify diagnoses to be used in care management and active medical management of treatment
Physical examination, medication review, and depression screening
Check vitals, conduct a physical exam that includes monofilament test, urine dipstick, and foot exam (as appropriate)
Communicate findings in your assessment that will be used to inform the PCP of potential gaps in care
Educate members on topics such as disease process, medication, and compliance
Comply with all HIPAA regulations and maintain security of protected health information (PHI)
General Responsibilities of the Clinical Advisor:
Perform Ride Alongs on assigned Housecalls Practitioners
Coaching and Evaluation of NPs with HouseCalls visits
Mentor and support new hires
Partner with CTMs to ensure clinical practice adheres to HouseCalls clinical model
Communicate findings to CTM both written and verbal
Utilize advanced clinical nursing expertise, knowledge of geriatric/chronic disease management to provide coaching, mentoring and role-modeling to new and existing NPs
Identify and support developmental clinical needs of NPs to achieve successful outcomes
Ensure appropriate implementation and adoption of clinical and quality initiatices as related to home visits
Foster and develop a culture of clinical expertise
Will continue to perform home assessments in addition to CA duties as determined based on team needs as outlined in touch category guidelines
Review and communicate RAL feedback to the HouseCalls practitioner
Collaborates with QA and Clinical Initiatives to identify individual and global needs for improvement
Works with Regional Director and CTM to determine percent of time in the field completing member visits
Positions in this job function require specific education/certification/licensure and are considered an extension to Physicians and provide direct patient care in a clinic or in-home setting

Current, unrestricted NP license in the state of assignment
Ability to obtain DEA licensure / Prescriptive Authority post-hire in states where applicable
Nurse Practitioner national certification as ANP, FNP, or GNP required
A clinical background in adult, family or geriatric specialties
Past experience working in a nursing home or with seniors in other settings
Computer literate and able navigate the internet
Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
A HouseCalls NP with a minimum of 1 year of field experience
You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.
If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas. In addition, employees must comply with any state and local masking orders
