In CanoHealth we are currently looking for a Nurse Practitioner who will, under the direction of a licensed physician, provide medical services to patients in our centers, using approved patient care protocols; collect and document data, conduct diagnostic and therapeutic procedures, order and schedule laboratory studies and professional consultations and provides direct patient care services.

Essential Duties & Responsibilities

  • Provides comprehensive clinical primary patient care through interviewing, examining and establishing treating plan for patients to meet their health and wellness needs.

  • Prescribes appropriate medical treatment and clinical medications consistent with Medical Standards for patients and orders studies, tests, and ancillary care services as required

  • Creates medically appropriate referrals

  • Follows all established rules and guidelines of evidence-based care and best clinical practice

  • Documents all services in electronic patient medical record

  • Regularly reviews chronic medical conditions that require ongoing medical attention on office visits.

  • Educates patients on wellness, prevention, and early detection by discussion and /or providing materials and resources to the patients and families

  • Provides routine office injections SQ, IM, or Intraarticular as per standards of care for primary care providers and Cano Health care standards.

  • Consults/collaborates with Cano Health Physician as needed regarding patient care, assessment, and education issues in patients with complicated medical conditions.

  • Maintains privacy and confidentiality of all patient information according to federal and state guidelines, regulations, and HIPAA.

Education & Experience

  • Master's Degree in Nursing

  • 3+ years' nursing experience

  • Valid nurse practitioner license

  • Ability to work within a team and take direction

  • Must be familiar with the EHR medical charting system

  • Strong written and interpersonal communication skills

  • Experience with ECW highly preferred

The APRN develops medical diagnoses, appropriate treatment plan, prescribes medical regimen, and educates patients/family regarding diagnosis and plan of care. May perform approved procedures and order medications as indicated in the Delineation of Privileges. The APRN works directly with physicians, staff, administration, patients and families to ensure quality of care across the health system. The APRN is a clinical team leader that intervenes and manages deteriorating patients. The APRN integrates evidence-based practice and technological advances into their scope of practice. The APRN may participate in research initiatives, data collection, quality improvement, discharge planning, and coordination of patient care within the area of practice. The APRN participates in and facilitates patient throughput, optimal patient outcomes, optimal length of stay and improved patient satisfaction. Minimum 3 years of relevant experience required. Obtain medical staff credentialing and privileges. Compliant with state requirements for CEUs and licensure renewal. DEA License required as per specialty. If APRN does not have acute care certification, refer to BHSF Policy 701 titled Educational and training requirements to be credentialed to work in an acute care setting for Advanced Practice Registered Nurse (APRN, formerly ARNP).
