Job Summary:

The Cardiothoracic Inpatient Acute Care Nurse Practitioner employed by EHS provides selected medical/surgical services within the specialty of cardiothoracic and vascular medicine and any others as defined by the Collaborative agreement the Inpatient Acute Care Nurse Practitioner has with the collaborating physician. The services delegated to the Cardiothoracic Inpatient Acute Care Nurse Practitioner will be an extension of the Cardiology and CT surgery faculty's patient management. Services rendered by the Cardiothoracic Inpatient Acute Care Nurse Practitioner must be provided under the supervision, direction, and ultimate responsibility of a licensed Cardiologist and/or CT Surgeon. He or She must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. The Cardiothoracic Inpatient Acute Care Nurse Practitioner will assist with the planning, implementing and evaluating the plan of care for pre and post cardiothoracic and vascular surgery patients. Management includes but is not limited to, performing history and physical examination, ordering and interpreting appropriate diagnostic and laboratory tests and administering of pharmacological and non-pharmacological therapies in a cost-efficient manner. He or She will function in collaboration with the multidisciplinary cardiothoracic and vascular team and provide consultation and education to patients, families, and members of the multidisciplinary teams. Additionally, the Inpatient Acute Care Nurse Practitioner must identify opportunities for and contribute to the improvement of quality, safety, and cost, as well as patient and customer satisfaction.

Education Required:

Masters of Science degree in Nursing (MSN). Adanced Practice Nurse. Must have Acute Care NP license.

Experience Required:

ACNP: A minimum of one to two years of experience in an inpatient cardiothoracic care setting

Preferred: two-three years of experience in an inpatient cardiothoracic care setting

ACNP: Critical Care, Emergency Department or Cardiothoracic experience

  • Ability to perform bedside procedures including but not limited to-

  • IABP removal

  • arterial line insertion and removal

  • Intubation

  • central line insertion

  • chest tube placement and removal

  • wound vac placement and changes

  • cardioversions

  • ECMO cannulations

  • Open chest first assistant/ utilization of cart during ECPR

  • PleurX catheter placement, drainage, and removal

  • Perform Swan Ganz catheter placement and PA wedge pressure procedure

  • All aspects of cardiothoracic surgery to include vein harvesting and the following surgical procedures:

  • CABG's

  • Aortic Valve Replacements

  • Ascending Aortic Aneurysm Repairs and Replacements

  • Mitral Valve Repair with Ring Placements

  • Mitral Valve Replacements

  • Type A dissections

  • Convergent Procedures

  • MAZE procedures

  • Left Atrial Appendage ligations and Clip placements

  • Robotic Mid CAB

  • Robotic Mitral Valve repairs

  • Thoracic Surgery procedures including but not limited to: lung biopsies , VATS procedures, Lobectomy's, pneumonectomy's, foregut procedures including esophagostomy's'

  • Mandatory cardiac on-call including but not limited to- outpatient care management, ICU and cardiac step down acuity

  • Mandatory weekend additional shift coverage that includes patient care rounds, management, education, and discharge processes

Position Requirement(s): License/Certification/Registration

Required:

ACNP:

Licensed as an Registered Nurse (RN) in the state of Tennessee

Licensed as an Advanced Practice Nurse (APN) in the state of Tennessee

National certification in a specialty (such as ACNP, ANP, CCNS, FNP)

BLS completed prior to going to the unit/department and maintained going forward

Certificate of fitness from the state of Tennessee

Department Position Summary:

The Inpatient Acute Care Nurse Practitioner employed by EHS HLI provides selected medical/surgical services within the specialty of cardiothoracic and vascular medicine. The services delegated to the Inpatient Acute Care Nurse Practitioner will be an extension of the Cardiology and Cardiothoracic surgery faculty's patient management. Care of this subset of patients is a continuum from the time of admission through discharge or final disposition. Services rendered by the Inpatient Acute Care Nurse Practitioner must be provided under the supervision, direction, and ultimate responsibility of a licensed Cardiothoracic and Vascular Surgeon. He or She must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served. The Inpatient Acute Care Nurse Practitioner will work collaboratively with all Surgeons, ED physicians, Cardiologists, and the Hospitalists. He or She must demonstrate strong knowledge base of cardiovascular system; advance Nursing principles, strong leadership skills, organizational functions, policies, evidence based practice, AHA/ACC/STS guidelines, and new developments. They will assist with the planning, implementing, and evaluating the plan of care for pre and post-surgical CT patients. Management included but not limited to, performing history and physical examination, new surgical consults, ordering and interpreting appropriate diagnostic and laboratory tests, and administering of pharmacological and non-pharmacological therapies in a cost efficient manner. The APP will function in collaboration with the multidisciplinary cardiothoracic team and provide consultation and education to patients, families, and members of the multidisciplinary teams. Additionally, the Inpatient Acute Care Nurse Practitioner must identify opportunities for and contribute to the improvement of quality, safety, and cost, as well as patient and customer satisfaction. The APP will:

Maintain and update policies, procedures, and algorithms in keeping with current evidence- based practice

Serve as both a resource and role model/mentor to RN staff, surgical heart team and all peers

Keep current with all required Continuing Education

Provide patient/family teaching to improve outcomes and patient satisfaction

Role Specific responsibilities include but are not limited to:

Perform detailed History and Physical, consults, post balloon removal procedure dictations and discharge summaries

Order, interpret, and provide treatment as necessary: CXR, EKG, Laboratory studies, and other diagnostic radiologic studies

Provide staff education

Provide patient and family education

Provide physician referrals as needed

Recognize and treat acute changes in patient status

Collaborate with physician sponsor and ancillary departments as needed

Round on patients daily
