The well established Associates in Cardiology Care at Bayonne is seeking to hire a Nurse Practitioner to join the team. The Nurse Practitioner will work Full time and will need to have Acute care , cardiac telemetry experience -
Patient Care Duties: Perform age specific history and physical with appropriate follow up and documentation.
Analyze and organize the care of clinically complex patients/clients, including socio- cultural, cognitive, functional, nutritional and educational evaluation, leading to the development of a problem list and management plan. In collaboration with supervising physician assess, implement, monitor patient management plan using diagnostic and therapeutic modalities. In collaboration with interdisciplinary team determine, order select diagnostics and therapeutic interventions and regimens to facilitate patient care with attention to safety, cost, invasiveness, simplicity, acceptability adherence and efficacy.
Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.
