In CanoHealth we are currently looking for a Nurse Practitioner who will, under the direction of a licensed physician, provide medical services to patients in our centers, using approved patient care protocols; collect and document data, conduct diagnostic and therapeutic procedures, order and schedule laboratory studies and professional consultations and provides direct patient care services.

Essential Duties & Responsibilities

  • Provides comprehensive clinical primary patient care through interviewing, examining and establishing treating plan for patients to meet their health and wellness needs.

  • Prescribes appropriate medical treatment and clinical medications consistent with Medical Standards for patients and orders studies, tests, and ancillary care services as required

  • Creates medically appropriate referrals

  • Follows all established rules and guidelines of evidence-based care and best clinical practice

  • Documents all services in electronic patient medical record

  • Regularly reviews chronic medical conditions that require ongoing medical attention on office visits.

  • Educates patients on wellness, prevention, and early detection by discussion and /or providing materials and resources to the patients and families

  • Provides routine office injections SQ, IM, or Intraarticular as per standards of care for primary care providers and Cano Health care standards.

  • Consults/collaborates with Cano Health Physician as needed regarding patient care, assessment, and education issues in patients with complicated medical conditions.

  • Maintains privacy and confidentiality of all patient information according to federal and state guidelines, regulations, and HIPAA.

Education & Experience

  • Master's Degree in Nursing

  • 3+ years' nursing experience

  • Valid nurse practitioner license

  • Ability to work within a team and take direction

  • Must be familiar with the EHR medical charting system

  • Strong written and interpersonal communication skills

  • Experience with ECW highly preferred

The Wound Care NP provides wound care services in accordance with applicable federal, state, regulatory and company standards. The Wound Care NP provides direct and indirect patient care in the clinical setting; evaluating and managing edema and wounds caused by disease, accidents, or surgical incisions. The Wound Care NP assesses and monitors patient progress during the healing process, and educates the patient on the care of wounds outside of the clinical setting. The Wound Care NP's performance will be tracked and comparatively reported using metrics related to the strategic direction and operational needs of the organization. Primary Responsibilites: * The Wound Care NP is expected to be able to provide care consistent with their licensure and/or certification. * Provides diagnosis, treatment, therapy and management of patients according to current standards of care * Advocates for patients and their families in providing them resources for care. * Tracks patient status; adjust wound management treatment based on standard wound care protocols. Documents progress in EMR. * Utilizes managed care processes for cost-effective and efficient use of lab, radiology and extended service providers, including specialists, hospitals, skilled nursing providers, and other ancillary services * Completes timely and comprehensive medical documentation in EMR systems according to company standards * Participates with peers, employed and DNG PCPs, for the comprehensive care of the WellMed patient. * Conducts or directs education effective communications to patients, families, and other health care providers * Conducts educational activities for organization's physicians, physician assistants, nurse practitioners, and their ancillary staff for the purposes of education and patient care. * Participates in business and patient care meetings * Performs all other related duties as assigned You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: * Graduate of an accredited Nurse Practitioner Program * Masters of Science in Nursing * Current and unrestricted licensure to practice in the state of employment * Current prescriptive authority from the state of employment * Possess a working understanding and support of a managed care environment * Ability to communicate effectively with all levels of employees and maintain relationships with various health plans * This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease. * You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role * Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe.
