Description: Hiring a Nurse Practitioner, Behavioral Health for a large physician led outpatient medical office in the New York Metro Area. The Nurse Practitioner, Behavioral Health, will provide medical services in collaboration with its primary care physicians.
Provide high quality patient care and a full scope of primary care services including diagnosis, treatment, coordination of care, preventive care and health maintenance.

Obtains and documents patient history as appropriate
Conducts relevant physical exam and evaluates patient, documenting pertinent positive and negative findings.
Diagnoses and treats disease and manages chronic illness.
Prescribes medication in accordance with New York State and federal law.
Provides preventative health services, including health education and counseling.
Collaborates with other clinicians and staff members to ensure continuity of care

Bachelors Degree required, Masters Degree preferred.
Graduate of a Nurse Practitioner education program registered by the New York State Education Department.
At least one year of experience as Nurse Practitioner
Current NCCPA Board Certification from the National Commission on the Certification as a Nurse Practitioner from a New York State approved accrediting body (ANCC) or be eligible for board certification.
Current and unrestricted licensed Nurse Practitioner by the New York State Department of Education or be eligible for licensure.

Experience working with diverse populations in an ambulatory healthcare setting.

DUTIES AND RESPONSIBILITIES (specific to Regional APNManager): * Mentor to NPs /CNMs, willcover the sites in Queens (LIC, FHC, Queens). * Willreview new graduate APN's (Less than 1 year) charts every 2 weeks first monththen every month for 5 months (10 charts each) and discuss with ANP * Willbe accessible via cell phone to answer clinical questions for new graduates * Teach portion of fellowshipclasses in didactic portion of NP fellowship * Assist in buildingcontent/classes for mini residencies (Pediatrics, HIV, TG, Prenatal) forcurrent NPs and CNMs * NP/CNM student preceptor * Help with Orientation ofnew hires Essential Functions: * Obtains and/or reviews/evaluates a patient'smedical and psychosocial history. * Performs complete comprehensive physicalexamination. * Performs screening procedures including Pap Smears,GC cultures, Chlamydia tests and Wet preps. * Facilitates related services for health centerpatients as appropriate with respect to their * confidentiality and privacy. * Performs routine interim physical examinations onpatients that are risk specific, and as outlined * in the agency's Medical Policies and ProceduresManual. * Evaluates individual patients and prescribesappropriate medications, treatments, referrals as * indicated in the Policies & Procedures Manualand/or is consistent with current standards of * care. * Provides continuous care to patients and managementof common and important clinical * problems encountered in a primary care setting. * Makes referrals to appropriate sources/consultantsfor medical and/or social problems. * Participates in patient education and counselingservices, especially about family planning, * STD'S/HIV, preventive health and nutrition. * Interprets laboratory data. Reviews and initialsall lab reports; orders appropriate/additional lab * tests and referrals or follows up as indicated.
