Overview
The Advanced Practice Provider (APP) is an integral part of the delivery of health care and provides for primary care in many ambulatory settings through the CHS Physician Enterprise. The APP provides patient care under the supervision of a qualified physician and is credentialed through the Medical Staff process.
Job Details
A Nurse Practitioner is responsible for handling patient appointments and performing routine examinations to ensure patient health. The duties include speaking with their patients about healthy lifestyle choices for their age, completing diagnostic tests to help diagnose and treat patients for injuries or illnesses and coordinating with other healthcare professionals to maintain updated medical records.

DUTIES/RESPONSIBILITIES:
Conducts comprehensive or episodic health history and physical assessment of patients with acute, chronic and/or potential health problems in the patient independent home or assisted living facility.
Collaborates with Medical Provider and affiliated physicians to establish medical diagnoses and treatment plans
Provides for continuity of care between clinic visits
Assigns specific orders within the limits of the practice agreement/scope of practice
Provides appropriate direction to clinic staff regarding patient care issues.
Participates in a team model of care delivery to identified group/panel of patients.
Meets regularly with appropriate physicians; works collaboratively to effect programmatic changes and develops protocols.
Participates in quality assessment and improvement activities. Identify important aspects of care and assure findings shared to improve patient experience and practice operations.
Works in collaboration with staff to provide on-going patient care and management
Holds responsibility for timely and accurate written documentation or patient care encounters/procedures.
Participates in Quality Assurance meetings

POSITION REQUIREMENTS AND QUALIFICATIONS:
Graduate of an accredited school of nursing. Bachelor Degree required
Graduate of an accredited program for Certified Nurse Practitioner.
Current licensure by N.Y.S. Dept. of Education to practice as RN and NP
Master's Degree and Certification required.
BLS Required
Physical Requirements:
Must be able to drive between system entities and patient home residence and/or assisted living facility.
Experience:
3 years' experience as a practicing Nurse Practitioner
Primary Care experience required
Salary Range
USD $66.55 - USD $93.81 /Yr.
