Job Shift: Shift 1

Shift Details: Weekdays, M-F

Hartford HealthCare is seeking a full-time APRN or PA for a primary care position in a large, multi-provider location in Wethersfield, CT.

The ideal candidate will possess:

  • A passion for providing patient-centered primary care focused on quality, prevention and education

  • At least one year of post-graduate advanced practice experience preferred

  • A willingness to work closely and collaboratively within an interdisciplinary team

  • Unrestricted State of Connecticut APRN or PA license

Position details:

  • Full-time, weekday hours

  • No weekends

  • No on-call

  • Robust and highly-trained support staff

  • Full-scope primary care medicine: patients 12+ years old; acute, follow-up and chronic disease management

Advanced Practitioner (APP) Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), performs routine physical examinations, and formulates medical, educational, and psychosocial plans to treat patient. In consultation with the physician(s), develops plan for further evaluation of health needs, e.g., referral to other health team members or specialists. Provides appropriate treatment for problems within scope of a Nurse Practitioner (APRN) or Physician assistant (PA) practice as designated by protocols. Identifies problems outside of APRN or PA scope of practice and refers appropriately to physician.

Requirements:

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program preferred

  • Unrestricted State of Connecticut medical license (or eligible)
