Hours Per Week:
Provides help with all aspects of patient care, including diagnosis, treatments and consultations as part of a physician/nurse team. Works within a Specialty Care team, in both inpatient and outpatient situations and can perform independently or as part of the treatment team ensuring continuity of patient care in all settings. Serves as an educator, case manager, consultant to staff and multidisciplinary health care team.
Provides clinical care for patients
Completes comprehensive patient assessments.
Determines diagnosis and treatment of illness.
Orders and interprets medical diagnostics in tandem with attending physician.
Prescribes medications, treatments, therapy and medical equipment.
Conducts patient education when working to manage chronic illness or wellness care.
Refers for specialty care when determined.
Consults with physician when patient needs outside of Nurse Practitioner scope of practice, diagnosis or treatment plan is unclear.
Participates in health care team
Promotes communication within the team and other interdisciplinary health care staff.
Acts as resource to members of the team.
Collaborates with hospital staff by consulting and rounding on patients.
Works with primary care team for program development.
Participates in research activities when opportunity available.
Acts as liaison with outside agencies as needed.
Other duties as assigned.
Master's degree Completion of accredited Nurse Practitioner program. required
2 to 5 years 3 years experience post Nurse Practitioner training strongly preferred. preferred

Additional Job Description:
2 positions available
Performs duties as part of the Podiatry Hospitalist program at Abbott Northwestern and United Hospitals. Assesses patients on admissions to the hospital and in the Wound and Vascular clinics. Performs and documents history and physical examination based on age and history on complex and chronically-ill patients.
Rounds on hospitalized patients in various settings including acute care to advance the plan of care, reassesses as patient condition warrants. Updates and records changes in health status.
Prescribes medication in accordance with state and federal regulations.
Submits billing levels at time of visit or procedure. Completes consultation, transfer and discharge summaries as needed.
Acts as a consultant for triage nurses; performs phone consultations with patients.
Acts as a liaison between Podiatry, Vascular, Infectious teams, other specialties and services along with outreach sites, providing staff consultation and education as needed.
Management of surgical inpatients, including wound care regimen and discharge planning
Outpatient care to podiatric patients in a wound clinic setting
Interest in wound care, including grafts and wound vacs
Work schedule will include weekend coverage.
