General Summary: An exempt position consistent with applicant#s certification, performs initial and ongoing assessment of patient#s medical, physical, and psychosocial status, including assessing, diagnosing, prescribing, treating and educating patients.# Works closely, with or beside physicians and other clinicians in a team approach to patient care.# May specialize in a specific type of clinical care, e.g., pediatrics, cardiology.


Essential Job Responsibilities:


#Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing appropriate physical examination. Orders/performs appropriate laboratory and diagnostic tests.# Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness.# Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks. Monitors and manages acute and chronic illnesses, performs procedures in accordance with their scope of practice and appropriate training/proctoring, performs wellness exams and Medicare Wellness exams, performs transition into care visits.# Delegates and instructs others appropriately to complete assigned responsibilities. Maintains continuity of patient care through patient follow-up, monitoring of patient progress and modifying plan of care as necessary. Development and implementation of treatment plans, prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws.# Instructs patient/family regarding medications and treatments.# Educates patients regarding health status, health promotion/illness prevention, disease processes. Recommends appropriate community resources to meet patient/family needs.# Communicates case management information to other professionals and community agencies. Prepares documentation for medical records including updating patient medical chart by recording examination and test results, diagnosis, medications, and treatment in written/computerized manner in accordance with the Standard Care Arrangement/Supervisory Agreement signed by both parties, to be provided to their director/manager and kept on-site where they practice. Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures.# Acts as consultant/collaborator to clinical team on area of specialty.# Complies with patient confidentiality requirements.# Promotes patient advocacy Rounds on patients in the hospital if a Credentialed employee through the hospital and may have call responsibility; this is specific to each practice. Exhibits knowledge of evaluation and management (E #M) coding guidelines, and demonstrates appropriate application of those guidelines Assumes responsibility and accountability for time management, professional relationships, and interpersonal interactions within the job setting, acting as a role model.# Precepts clinical staff and other Advanced Practice Providers


Education:


APRN: Successful completion of a Masters/post- Master#s degree in a program accredited by the Commission on the Collegiate of Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC) or an equivalent approved program, current certification by a national certifying organization approved by the Ohio Board of Nursing, and a current active licensure to practice as a CNP/CNS in the State of Ohio.


Current CPR certification PA:


Completion of an Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) approved program and current certification by the National Commission on Certification of Physician Assistants (NCCPA) and current license to practice as a PA issued by Ohio Board of Medicine.


Current CPR certification Experience:


Applicants must be able to demonstrate provision of care, treatment or services, reflective of the scope of privileges or a post master#s degree program in past 12 months. Physical Demands:


Requires standing and walking for extensive periods of time.# Occasionally requires lifting and carrying items weighing up to 10 pounds, requires working under stress in emergency situations and occasionally may involve working irregular hours.# Requires corrected vision and hearing to normal range.
