As a facility-based Advanced Practice Provider, you will be responsible for providing acute and ongoing patient care within a Post-Acute Care setting- diligently working towards enhancing the clinical status and quality of life of each patient. This involves conducting Annual Wellness Visits and may entail performing Annual History and Physical examinations as per the facility's requests. Your significant duties will also entail bearing the responsibility of coordinating patient care with collaborating physicians for overall medical management, maintaining communication with families, and attending family conferences whenever feasible.

The role may also entail other tasks as assigned, reflecting the fluid and dynamic nature of healthcare provision. Qualifications: The successful candidate must have a valid license as a nurse practitioner or physician assistant in the state of operations, along with board certification in their field. A minimum requirement for the role is a Bachelors and Masters Degree in Nursing from accredited institutions.

The ideal applicant will have experience in nursing and ideally some experience as a Nurse Practitioner. Exceptional critical-thinking skills and a good understanding of nursing principles, particularly in relation to aged, chronically ill, and convalescing patients, are key. Additional attributes beneficial for the role include strong leadership and problem-solving abilities, impressive written and oral communication skills, punctuality, organizational skills, and a cooperative team spirit.
