This role involves providing primary healthcare to seniors, allowing them to live in their communities and homes. By working under the Medical Director's direction, the Nurse Practitioner collaborates with a team of physicians and interdisciplinary teams to offer comprehensive medical services. This includes assessments, care planning, and continuous medical support to enhance participants' quality of life.

Required Qualifications: Master's degree from an accredited nursing program. Certified Nurse Practitioner with Prescriptive Authority.

Valid California NP State License. A minimum of two years' experience with the senior population. In-depth knowledge of chronic care and geriatric best practices.

Clean DMV record and valid state driver's license. Recent vehicle insurance copy. Preferred Qualifications:

Prior experience in care coordination and policy improvement is an advantage. Responsibilities: Conduct comprehensive, initial, semi-annual, and unscheduled assessments of participants. Collaborate with physicians and interdisciplinary teams to develop and implement care plans.

Participate in discharge planning and maintain participant care in various settings, including nursing homes. Review data on outcomes and utilization to guide best practices and policy enhancements. Engage in rotating night and weekend calls.
