Position/Job Title: Nurse Practitioner Job Number: 100200270 Employment Type: Fulltime Location: Tacoma, WA 98401 Industry: Healthcare Salary: $142,500.00 Per year Job Description: Our client is revolutionizing care for individuals with Serious Mental Illness (SMI) by focusing on whole-person care and improving outcomes. We are seeking a licensed Nurse Practitioner to lead the medical aspects of team-based care for our service users. This role involves performing comprehensive clinical assessments, leading medical components of care, and collaborating extensively with a multidisciplinary team to manage and coordinate care effectively. Primary Duties and Responsibilities: - Conduct whole-person clinical assessments, including gathering information, assessing health conditions, and making referrals.

• Lead medical component of team-based care, providing training and advice on clinical issues.

• Support care coordination among medical and behavioral health providers.

• Perform medication reconciliation and coordinate with care providers to ensure comprehensive care management. Qualifications: - Master's degree or higher in Nursing.

• Valid Nurse Practitioner license in the state of Washington.

• 5 years of clinical care experience, ideally in a primary care setting.

• Experience managing underserved populations with behavioral health and/or SUD conditions.

• Familiarity with trauma-informed care and social determinants of health.

• Strong computer skills and familiarity with Microsoft Office Suite.
