Responsibilities: - Embracing the client's philosophy of providing care to enhance each patient's clinical status and quality of life

• Conducting Annual Wellness Visits for long-term Nursing Facility residents and possibly performing Annual History and Physicals upon request

• Administering clinical interventional and episodic direct care visits, and re-evaluating visits as necessary

• Ongoing evaluation and treatment of chronic, co-morbid illnesses

• Review of medications, orders, and diagnostic tests, responding to clinical abnormalities for patients under care

• Care coordination with collaborating physicians for overall medical management and other disciplines like therapists, specialists, and social workers

• Family communication and attendance at family conferences if feasible

• Other assigned duties Qualifications:

• A valid state license as a nurse practitioner or physician assistant, along with current board certification

• A bachelor's and master's degree in nursing from an accredited institution

• Nursing experience is required, with preference given to those with some nurse practitioner experience. Critical thinking skills are a must

• Familiarity with the application of nursing and clinical Nurse Practitioner critical thinking skills to aged, chronically ill, and recovering residents

• Strong leadership, interpersonal, and problem-solving abilities are preferred

• Outstanding written and verbal communication skills
