The Nurse Practitioner will provide direct care and coordinate the care of Cedars-Sinai Health System patients with Alzheimer's disease and other forms of dementia. Participate in interdisciplinary neurobehavioral clinics, provide education and support and make referrals to other services/specialties as appropriate. Document and bill for services rendered accurately, timely and thoroughly.
Summary of Essential Job Duties:
Under the general supervision of the Medical Director, assist in developing evidence-based, collaborative care protocols for patients with Alzheimer's or other forms of dementia.
Maintain components of dementia population health and case management systems
Conduct structured patient/caregiver needs assessments
Develop and implement individualized dementia care plans based on needs assessments
Communicate care plans to physicians and other health practitioners to ensure continuity of care
Provide Intensive Dementia Care Management supervised by a physician dementia specialist, which include: a) in-person and/or virtual sessions at which patient and family members' specific questions about problems, resources, and implementing care plans are answered; b) telehealth/telephone follow-up to monitor implementation of dementia treatment and care plans; c) facilitation of appointments with consultants when the treatment plan needs to be reassessed (e.g., new neurologic, medical or behavioral symptoms); d) teaching dementia education, management and coping skills to caregivers through individual counseling including information on: legal and financial planning with referral to community services, behavioral techniques to avoid/manage behavioral problems, and coping strategies for caregivers
Refer, when appropriate, to community-based organizations for services and resources, participate in quality improvement efforts for patients participating in the Cedars-Sinai Health System Memory & Aging program
Collaborate and provide assistance in the education of physicians and nurses in training and training of clinical staff in other clinical areas including the emergency department, inpatient units and ambulatory care practices.
Master's Degree, Nursing (MSN) required
Graduate of an accredited Nurse Practitioner program required
3 years of Acute Care nursing experience preferred
Experience as an Advance Practice Nurse preferred
Experience working with Dementia and/or Alzheimer's patients preferred
