We're expanding and hiring a Full-Time Family Nurse Practitioner to work at our various locations.
What You'll Be Doing
As an FNP with the new Dorothy Perry Hall, you would be providing strength-focused, evidence-based and culturally sensitive treatment. Creating and sustaining positive, compassionate interactions with patients and providing high quality care will be central to the work you do. You would conduct assessments, manage cases, and provide supportive therapy with utmost kindness and care. Your attention to detail and problem-solving skills will be important to your responsibilities related to evaluating and managing medications. Furthermore, you'd have the distinct opportunity to work closely with a multidisciplinary team of Health Solutions professionals to serve our community.
Specifically, the FNP responsibilities under the administration of the Director of Medical Services will:
Administrative duties
Maintains accurate and current records with 100 percent compliance with Health Solutions' standards
Uses electronic health records and electronic prescribing accurately and facilely to promote patient care
Clinical duties
Conducts assessments and engages with patients with kindness and a focus on their needs
Prepares and administers prescribed medications, injections, and vaccinations
Facilitates timely and accurate medication prescriptions and refills
Maintains medications securely
Cares for wounds
Maintains and organizes medication records
Reviews laboratory results with the treatment team and patients
Applies best practices in medical care and medication assisted treatment
Monitors chronic disease markers and reviews them with the treatment team and patients
Uses developmental milestones in assessing and caring for patients
Offers psychoeducation to patients and their family about topics relevant to high quality care (e.g., disease processes, sleep hygiene, nutrition, behavior modification)
Implements preventive medical interventions in collaboration with patients and healthcare specialists
Evaluates behavioral health assessments and makes referral to behavioral health professionals
Witnesses urine toxicology screenings and performs oral drug testing
Maintains productivity
Care integration and collaboration
Participates in and effectively collaborates with our multidisciplinary teams of professionals to promote the best quality care
Participates as a valuable team member in meetings and consultations with other members of the team

Qualifications
What We're Looking For-The Must-Haves
Current license as a Family Nurse Practitioner in Colorado
Graduate degree in nursing from an accredited academic institution
Two years of experience as FNP
Current Colorado driver's license
Access to reliable transportation during work hours
Computer proficiency
What We'd Like to See in You-The Nice-to-Haves
Experience caring for individuals with addictions, and severe and persistent mental illness.
Track record of working successfully as a part of a multidisciplinary team
Experience in managed care environments
Spanish language skills
