We are seeking a Behavioral Health Nurse Practitioner to join us in pursuit of high quality patient care, providing full-scope psychiatry services. On our team, you would be our partner in an exciting growth opportunity as we implement and build an expanding behavioral health service that is innovative in delivering integrated care as an extension of the high-performing care provided at our medical care centers.
ResponsibilitiesThe Ideal Candidate will have:
A passion for addressing behavioral health issues in the primary care medical setting and serving the behavioral health needs of patients with comorbid medical conditions with an integrated approach to care
Independent motivation and drive to facilitate service growth and guide population-based care for behavioral health
Desire to work in a professional team setting with clinicians in primary care and colleagues in behavioral health
Excellent professional competence and collegial rapport adeptness, including openness and flexibility
Experience working with additional populations of children, adolescents, and substance use disorders are highly desired.
Integrated care experience, knowledge, familiarity or interest a plus, particularly in consultation for guiding care with behavioral health colleagues and primary care providers and with a focus and interest in the ambulatory care setting
Essential Duties and Responsibilities:
As a practicing clinician, provide full scope of psychiatry services including but not limited to diagnosis and treatment of common acute psychiatric problems, illness, and crises and facilitates in coordination of care, preventive care, and health maintenance.
Provide outpatient care and counseling for patients with chronic or urgent medical conditions showing a change or decline in mental status.
Take part and guide consultation in an interdisciplinary team within primary care working with behavioral health colleagues to serve patient's needs.
Conduct comprehensive psychiatric diagnostic evaluation including psychosocial, developmental, functional, and risk factor assessment, develop a differential diagnosis, and order of therapeutic and diagnostic tests as needed
Attend to consultations and appointments to enhance continuity of care.
Develop, implement, and monitor therapeutic plan of care, including prescription of medication as well as other interventions in collaboration with behavioral health colleagues.
Track patient response and progress to treatment plan and modify as needed toward improved outcomes for patients behavioral and overall health.
Psychopharmacologic management in collaboration with psychiatrist.
Prescribe and manage medications following the clinic's policy, formulary and relevant regulations of the State.
Review with collaborating psychiatrist regularly and as needed for specialty input on cases and care needing further guidance or support, and consultation for those that are beyond the scope of qualification or expertise.
Collect, record, and maintain patient information, such as medical histories, reports, and examination results. Complete and ensure accuracy of patient medical records.
Help to monitor common health care problems and refer for specialized medical treatment as needed and appropriate
Facilitate arrangements for patient referrals or consultations in collaboration with primary care as appropriate.
Communicate effectively with the clinical team to ensure total delivery of quality care.
Ensure smooth interchange with primary care team members for efficient delivery of clinical services.
Provide direct education about behavioral health care in the clinical setting and in more structured didactic settings as well when needed.
Participate in medical staff educational programs and meetings.
Identify, document and evaluate patient and family teaching needs. Work with patients and their families providing education about mental illness and behavioral health treatment and prevention.
Effectively promote high standards of practice through commitment to implementing and adhering to policies, protocols, and practice guidelines.
Education & Experience
Master of Science Degree in Nursing
Board Certified Psychiatry Nurse Practitioner (PMHNP)
Must possess unrestricted New York State Mental Health Registered Nurse License
Infection Control Certificate
Must possess Drug Enforcement Agency (DEA) registration or be eligible for registration
Must possess Medicare/Medicaid Enrollment or be eligible to enroll
3+ years of related clinical experience
Must possess strong communication skills both oral and written
