Wentworth-Douglass Hospital (WDH), a subsidiary of Massachusetts General Hospital, is currently recruiting for a RN Nurse Navigator with Cardiology experience to join our Heart Failure Team in Dover, NH. Reporting to the Practice Manager, some responsibilities may include:

  • Functioning on the multidisciplinary team as an advocate, interpreter, educator, and counselor for the cardiology patient.

  • Assisting with ensuring all adult patients within the defined specialty with a known or suspected heart failure diagnosis receive quality and comprehensive service.

  • Providing expert nursing care when appropriate which will include direct clinical practice, consultation, education, and research.

  • Coordinating patient care throughout the continuum in collaboration with the multidisciplinary team acting as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with clinical and supportive services.

  • Serving as a clinical resource with expertise in the defined specialty.

The Heart Failure (HF) Nurse Navigator is a professional whose clinical nursing expertise guides patients, families and their caregivers to informed decision-making, collaborating with a multidisciplinary team to allow for timely heart failure screening, diagnosis, treatment and increased supportive care across the continuum. The navigator will enhance patient access and experience through multidisciplinary team collaboration, acting as a single point of contact, providing for more comprehensive, well-coordinated and personalized heart failure care.

The HF Nurse Navigator assumes responsibility and accountability for coordination of care for a defined patient population by cardiology specialty to achieve optimal patient outcomes benefiting the patients, their physicians and family members. The navigator assists with collection and analysis of metrics to evaluate and direct the program goals including quality, service, patient satisfaction, appropriateness of care and cost effectiveness. The navigator serves as the key director of care within the specialty program.

Wentworth-Douglass Hospital includes 178-bed Magnet Recognized hospital, several urgent care and walk-in care facilities, multiple testing centers, 30 Wentworth Health Partners primary care and specialty care provider practices, The Works Health and Fitness Center and the Wentworth-Douglass Foundation. Schedule/Hours: Full-Time; exempt.

Minimum Experience Required:

  • At least five years Nursing experience.

  • At least three years Cardiology Nursing experience preferred.

Minimum Education Required:

  • RN with eligibility for BSN and enrollment in BSN program.
  • BSN/MSN preferred.

Minimum Licensure Required:

  • RN in State of New Hampshire .
  • AHA BLS.
  • CHFN Certification preferred.

The Preadmissions Nurse Navigator serves as the key director of a specialty specific multidisciplinary program, developing and coordinating the model of care and services for the patient population. a. Develops program standards for the operation of the center and acts as the coordinator of patient throughout and optimization activities. Develop, implement and maintain initial screen of surgical candidates to determine the scope and depth of additional patient assessment and treatment required to minimize patient risk and harm.
