Join a world-class academic healthcare system, UChicago Medicine Crown Point, located in Crown Point, Indiana as a Nurse Navigator. UChicago Medicine is an integrated academic and community health system with multiple primary medical facilities within the surrounding suburbs of Chicago. UChicago Medicine, Crown Point is a new two-story, 130,000-square-foot multispecialty care center and micro-hospital in Northwest Indiana for what will be the academic health system's largest off-site facility and its first freestanding building in Indiana. It is anticipating an opening in Spring 2024.
The Crown Point care center will include:
A micro-hospital with an eight-bed emergency department and a short-stay inpatient unit
A comprehensive cancer center with infusion therapy as well as radiation, medical and surgical oncology
An imaging center with MRI, CT, PET, X-ray and ultrasound capabilities
An outpatient surgery center
Laboratory services
Medical offices with access to UChicago Medicine's physicians and specialists, including those in cancer care, cardiology, digestive diseases, orthopaedics, neurosciences, pediatrics, primary care, surgical specialties, transplant care and women's health
Job Summary:
In this role you will partner with the rest of the clinical team to provide patients the highest quality of care.
Assists physician in clinic or procedure area with patient examinations, diagnostic tests and/or therapeutic procedures
Counsels patients concerning lifestyle modifications, medication regimens, side effects, and interactions using an appropriate psychosocial communication level
Communicates to clinics coordinators follow-up appointments, ancillary and/or pre-operative testing and scheduling needed for patient
Arranges patient admissions; coordinates ordering of tests/procedures with fellows, attending, and other nursing staff
Assists physicians in coordination of research programs by screening patients for acceptance into clinical research and outcome programs. Maintains office file notes, reviews charts, enters data for sectional demographic data base, assists in summarization and analysis of data

Essential Job Functions:
Conducts, responds to, follow up on, and triages patient phone calls to the physician as appropriate
Collects laboratory samples and performs lab testing in accordance with CLIA regulations and established policies and procedures. Demonstrates ability to perform laboratory Quality Controls and Assessments, and routine daily, weekly, monthly and periodic maintenance and function according to Laboratory policy and procedures
Participates in QAPI activities including participation in accreditation preparation and survey process and maintenance of accreditation, state, and CMS standards. Attends and participates in all applicable facility meetings, training, and development workshops, and reviews/understands all required policies and procedures
Ensures proper supplies, appropriate medical equipment and medications are available for patient care and par levels meet patient and facility needs
Qualifications:
License/Certifications:
A Bachelor's degree in Nursing
Must be an actively licensed Registered Nurse in the state of Indiana
Skills:
Minimum of two years' experience as a registered nurse in a clinical setting.
Strong verbal and writing skills.
Strong interpersonal skills; ability to handle a wide variety of public contacts, including conflicts with patience, tact, courtesy, and discretion.
Demonstrated ability to organize a demanding work load.
Ability to function both autonomously and as part of a team.
Considerable skill in determining priorities, working independently, and making decisions.

Demonstrated ability to exercise judgment, make independent decisions, and assume responsibility.
