Summary

Facilitates a timely, integrated and coordinated approach to the care of patients. Uses a multidisciplinary approach and acts as a liaison between the patient, staff and referring physician offices as well as ancillary areas of the hospital system.

Identifies learning needs and content for educational programs. Develops, presents, coordinates and evaluates educational activities. Effectively performs in the role of clinician, facilitator, advocate, consultant and/or researcher.
