Job Summary:

Interim HealthCare, is seeking a talented Hospice Case Manager, Registered Nurse for a great hospital in Warsaw, Virginia. This is a full time contracted position with a pay rate of $52 per hour with day shift availability. You must have 2 years of recent and relevant experience.

Responsibilities:

Assess, develop, and implement patient care plans based on individualized needs.

Monitor and record vital signs, symptoms, and medical history of patients.

Administer medications and treatments, and assist with diagnostic tests and procedures.

Provide education and support to patients and families regarding their medical conditions and treatment plans.

Maintain accurate and up-to-date documentation of patient progress and care in electronic medical records.

Collaborate with interdisciplinary healthcare team members to coordinate patient care and ensure optimal outcomes.

Adhere to infection control, safety, and quality standards and protocols.

Requirements:

Active Registered Nurse (RN) license in the state of Virginia.

Minimum of 2 year of recent experience in OUTPATIENT HOSPICE setting.

Current CPR certification.

Strong clinical and critical thinking skills.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a team.

Must be able to lift and move up to 50 pounds.
