Position Summary and Job Duties
This position is responsible for nursing services at Rock Haven by providing clinical and managerial leadership and supervision for the day to day operations of 64 residents living in two 32 bed neighborhoods. Coordinates care and services per guidance from ADON and DON in accordance with the current policies, federal, state and local standards, guidelines and regulations. Provides weekend coverage for the entire facility as needed. Appraise performance, rewarding and disciplining employees, address complaints and resolve problems.
80% Coordinates the care of residents/clients on assigned shift.
Receives report from NOC supervisor during the 0600-1430 week and gives report to the 2nd nurse manager coming in at 0730-1600 that week. Also receive report from nursing staff throughout the shift and gives report to on-coming shift supervisor.
Communicates shifts concerns by unit to appropriate staff (e-mails, voice mail etc).
Serves as administrative authority for the smooth function of assigned unit.
Notifies Administrator, DON or ADON of urgent concerns during the shift. (When the concern is non-urgent, notifies by phone message or e-mail).
Performs rounds at least twice each shift to monitor the conditions and care of all residents and assist staff as needed.
Completes wound rounds on the unit with assistance of the ADON as needed.
Assists RNs and LPNs to assess residents who are experiencing acute changes.
Provides clinical expertise in the medical management, psychiatric emergencies, crisis and behavior management techniques.
Responds to Code Blue, Medical Assist and Code Green situations. Assists staff as needed.

Supervises staff and assures regulatory compliance on a daily basis. Provides education, guidance and support to staff daily.
Initiates investigations for all alleged incidents of abuse, neglect or misappropriation of resident belongs. Maintains administrative records.
Audits charts routinely. Provides clinical expertise in documentation including MDS, Medicare, acute illness/change of condition charting, summary charting and care planning.
Participate in the Nutrition At Risk Program for you unit.
Participate in assigned QAPI's.
Work with education nurse to provide on-going education/competancy testing, to the nursing staff on designated unit.
Ensure that Roster Matrix 802 and 672 is updated daily and turned in weekly.
Facilitate and organize admissions to designated unit.
Manage day-to-day infection control monitoring
Provides nursing tasks during staff shortages.
Participates in a weekend supervisor coverage rotation and on-call rotation on the weekends worked as described in the Job Requirements below.
10% Quality Assurance and Regulatory Compliance:
Meets routinely with the DON, ADON, nurse manager group and the interdisciplinary teams.
Represents nursing on the Rock Haven Quality Assurance Committee.
Assists with the development and implementation of Nursing Policies and Procedures.
Directs their units nursing department in regulatory/survey compliance, quality assurance and staff development activities.
Reviews injury and risk forms. Assures that preventive measures have been initiated.
Initiates and reviews investigations for all alleged incidents of abuse, neglect or misappropriation of resident belongs. Maintains administrative records. Notifies the ADON/DON/NHA and reports to the State per regulation.
Receives employee, resident and family grievances. Refers information to appropriate supervisor. Works with NHA to assure that grievances are resolved.
Directs staff response to emergencies such severe weather, fire or bomb scare. Participates in fire drills. Directs disaster evacuation plan as needed.
Participates in state and federal nursing home survey process and assists in developing, implementing and auditing plans of correction.
5% Public Relations:
Provides tours of the facility to families in absence o the admissions coordinator.
Complete referral forms for any inquiries/tours in the absence if the admissions coordinator and reviews referrals as requested by ADON/DON to determine medical and nursing needs.
Accompanies the admissions coordinator as necessary for on-sights of referral to assess the clinical needs and appropriate placement of potential new admissions.
Represents the facility at community events.
Collaborates with appropriate community and governmental policy making bodies.
5% Performs related work as required.

Job Requirements, Education, Training & Experience
Registered Professional Nurse with current Wisconsin Licensure in good standing.
Supervisory experience in a team leader role.
Work experience as a professional nurse, preferably in related fields (long-term care, medical-surgical nursing, mental health or nursing supervision).
Skill with Microsoft Office, American Data ECS, Groupwise (group email) and Personality (scheduling program) software programs.
Provides every other weekend Supervisor coverage AM shift in a shared rotation with the 2nd Nurse Manager and the ADON. The person working the weekend will flex their schedule to be off the Friday before and Monday after the weekend worked. The 2nd Nurse Manager and ADON will cover clinical management of the unit on these flexed days off (Monday & Friday) for the weekend.
Works a rotating schedule with the 2nd Nurse manager of 0600-1430 one week and 0730-1600 the opposite week.

Rock County is located in south Central Wisconsin along the Illinois border. The County has a population of 161,188 making it the ninth largest County in Wisconsin. Janesville, Wisconsin (pop. 64,009) is the county seat. Janesville is located on the I39/90 corridor, forty miles south of Madison, seventy-six miles west of Milwaukee and about two hours from Chicago.
Rock County's many historic and cultural attractions include the City of Evansville historic district, Beloit College (an acclaimed liberal arts institution located in the City of Beloit) and the City of Janesville's Rotary Botanical Gardens and Tallman House (an exceptional example of Italian villa style architecture from the mid 1850's).
The Cities of Janesville and Beloit have a combined symphony orchestra, as well as individual performing arts centers. The Rock County Fair and Riverfest are regional cultural celebrations held every summer in the Cities of Janesville and Beloit respectively. These Cities both have extensive park and open space networks, including portions of the Ice Age National Scenic Trail, a Statewide trail commemorating the State's geologic history. Additionally, the Rock River runs through these Cities, offering many recreational opportunities.
Employees are paid every two weeks on Friday (26 checks in a year). Paychecks are directly deposited in the employee's bank account. There is a two week delay between the pay period worked and the payday. New employees generally wait four weeks before they receive their first check.
Employees receive vacation based upon the work group/bargaining unit their job class is assigned to.
Most employees receive the following holidays: 1) New Year's Day; 2) Spring Holiday to be observed the Friday immediately preceding Easter; 3) Memorial Day; 4) July 4th; 5) Labor Day; 6) Thanksgiving Day; 7) Friday after Thanksgiving Day; 8) Day before Christmas; 9) Christmas Day; and l0) one Floating Holiday of the Employee's choice. Rock County offers a dual choice point of service plan. Employees will have a choice to elect coverage with either Dean health Care of Mercy Health Care.
Sick leave pay shall commence on the first day of any period of illness due to accident, injury or disease.
All full-time employees shall earn one sick leave day per month of continuous employment. All part-time employees whose regular workweek is sixteen hours or more shall earn one sick leave day on a prorata basis directly in relation to the normal full time employment period. All part-time employees, who work less than sixteen hours per week, shall not earn sick leave. Temporary and seasonal employees are not eligible for sick leave.
Sick leave shall be granted after three months continuous service (from original hire date).
Sick leave shall accrue to a maximum of one hundred thirty days.
The County offers a dental plan through Delta Dental and will pay 60% of the applicable
premium for the lowest cost plan. Employees can enroll within the first 30 days of
employment, after that there is an annual open enrollment period in the fall.

The County provides term life insurance through the State of Wisconsin Plan. Employees can purchase insurance in increments based on their salary, up to five times their salary. The County will contribute 20% of the cost for the first increment only - one times the employee's salary. Employees can also purchase coverage for their spouse and/or children.
The County offers voluntary benefits which employees can enroll in at their own cost. Employees can enroll within the first 30 days of employment, after that there is an annual open enrollment period in the fall. Please see the enclosed voluntary benefits packet for more information.
Employees can participate in one of several deferred compensation plans (Nationwide, Wisconsin Deferred Comp, and Security Benefits) at their own cost.
Employees can elect to put an annually established amount into a flexible spending account through (EBC) to pay for their own non-reimbursed medical care expenses. Employees can also put in up to $5,000 for dependent care accounts.
If you leave County employment, you have ninety (90) days to submit expenses incurred prior to your last day.
Rock County participates in the Wisconsin Retirement System (WRS). Employee who have questions about their retirement benefits should contact WRS. What an employee receives at retirement is dependent upon a number of factors - age, length of service, salary.
Rock County and the employee make equal contributions to the Wisconsin Retirement System on behalf of the employer. The contribution rate is addressed in the employment letter.
Employees in protective status have the percentage contributions defined in their respective collective bargaining units.
In the event of a death an employee may be excused from work without loss of pay for up to a maximum of thirty-two (32) hours annually for the purpose of attending a person's wake, visitation, memorial service, funeral, or make necessary arrangements regarding the person's death, within a reasonable time after the occurrence.
If additional time is required beyond the thirty-two (32) hours annually, an employee may request to use accumulated vacation, holiday or comp-time. Sick leave cannot be used.
Bereavement leave cannot be accrued from one year to the next.
Bereavement leave can be used in increments of quarter hours.
All leaves under this section shall be prorated based upon the employee's FTE. Personnel Ordinance 18.509.
The County provides an Employee Assistance Program (EAP) for employees to use for all kinds of personal and professional assistance. In the case of counseling, professional counselors are available 24 hours a day, 7 days a week. An employee can establish a relationship with a counselor over the phone or set up an in-person appointment with a counselor in the area. The EAP program will pay for up to three visits.
The County provides free parking at all of its facilities.
All County buildings are smoke free. By County Ordinance anyone visiting or working in any County building is prohibited from smoking closer than 30 feet from any building entrance. The Ordinance also makes the entire Rock Haven Nursing Home a smoke free "campus."
