Nashville Community Care & Rehabilitation at Bordeaux is a 419 bed skilled nursing facility situated on a park-like campus on the northwest side of Nashville, Tennessee. The elegant building, dating back to the 1800s, is immaculate and provides highly skilled clinical care.

We also have a unit dedicated to ventilator dependent patients, vent weaning and tracheostomy patients. Professional therapists provide physical, occupational, and speech therapies in 3 spacious gyms, while restorative therapy is offered in another large gymnasium. Many other services are available for a wide assortment of conditions requiring round-the-clock nursing care, including a secure unit for individuals with advanced dementia.

As a Registered Nurse (RN) or Licensed Practical Nurse (LPN) for our team, you will be responsible for providing direct and compassionate nursing care to our residents. Additionally, this position will include supervision of the day-to-day nursing activities performed by nursing assistants in accordance with current Federal, State, and Local standards, guidelines, and regulations that govern our facility, and as may also be required by the Director of Nursing to maintain the highest degree of quality care at all times.

Requirements for Consideration include:

  • Registered Nurse/Licensed Practical Nurse with current state license.

  • Must have current/active CPR Certification.

  • Confirm that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.

  • Excellent technical, assessment, and documentation skills.

  • Customer service oriented with the ability to work well under pressure.

  • Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.

The RN is responsible for assessing, planning, implementing, and evaluating total patient care and nursing care plans for each patient. Responsibilities * Obtains a medical history and assesses, the patient's physical and emotional status, level of competency, home environment, safety factors, family or household member's ability to assist with care and the needs of the patient and what teaching will be required. * Serves as the patient's advocate by providing information on patient rights, grievance procedures, reimbursement procedures, and patient/family responsibilities. Involves patient's family in developing the Plan of Care. * Participate's in updating the interdisciplinary care plan every two weeks including the means of implementing the correct interventions to attain goals. * Supervises and directs the care given by the Hospice Aide/CNA & evaluates such care in accordance with guidelines and regulations. * Observes signs, symptoms and reactions to treatments, and informs the attending physician and/or Patient Care Coordinator (PCC) immediately of any changes. * Participates in Performance Improvement activities. * Participates in on-call rotating schedule and provides back-up assistance as requested. * Willing and able to accurately utilize any automated device required to document in the electronic medical record. Qualifications Must be a Registered Nurse licensed in the state of operation. Nursing experience required, with experience in Hospice or Home Health preferred.
