Overview

At St. Luke's, our dedicated team of Hospice Nurse Liaisons strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences.

Nurse Liaisons are a critical role for the success of all post-acute care. A successful liaison will have great people skills, be able to build trusting relationships with hospital staff and providers and have a true passion and desire to care for end-of-life patients to ensure their needs are met. You will coordinate care within the Treasure Valley area.

What you can expect from this role:

You will be responsible in coordinating end-of-life care for patients. Much of the Liaison's time is spent performing assessments and screening, establishing patients' eligibility for care, reviewing referrals for appropriate placement, meeting with patients/families to describe our services and educate them on the benefits of home care. You will also work closely with providers and staff for other possible referrals.

  • Full Time Days (Mon-Fri 8 hour shifts) with occasional on call

  • Fosters the relationship between patients and the facilities providing their care

  • Collaborate to design individualized care for adult patients

  • Represent the organization and participate in marketing and education programs to increase awareness across the system

Qualifications

  • Education: Nursing Degree

  • Experience: 2 years of RN experience; Hospice or Acute Care experience helpful

  • Licenses/Certifications: Current RN License in the State of Practice. Current Basic Life Support (BLS) Certification

  • Valid Driver's License and proof of Auto Insurance
