At St. Luke's, our dedicated team of Hospice Nurse Liaisons strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences.
Nurse Liaisons are a critical role for the success of all post-acute care. A successful liaison will have great people skills, be able to build trusting relationships with hospital staff and providers and have a true passion and desire to care for end-of-life patients to ensure their needs are met. You will coordinate care within the Treasure Valley area.
What you can expect from this role:
You will be responsible in coordinating end-of-life care for patients. Much of the Liaison's time is spent performing assessments and screening, establishing patients' eligibility for care, reviewing referrals for appropriate placement, meeting with patients/families to describe our services and educate them on the benefits of home care. You will also work closely with providers and staff for other possible referrals.
Full Time Days (Mon-Fri 8 hour shifts) with occasional on call
Fosters the relationship between patients and the facilities providing their care
Collaborate to design individualized care for adult patients
Represent the organization and participate in marketing and education programs to increase awareness across the system

Qualifications
Education: Nursing Degree
Experience: 2 years of RN experience; Hospice or Acute Care experience helpful
Licenses/Certifications: Current RN License in the State of Practice. Current Basic Life Support (BLS) Certification
Valid Driver's License and proof of Auto Insurance
