ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

The Household Nurse Leader provides clinical leadership to the household ensuring the mission, vision and values of the organization are supported through the clinical services of the household. The Household Nurse Leader, working in concert with the Household Coordinator, plans, organizes, directs and facilitates the clinical operations of the household to ensure regulatory compliance, nurture a person-centered and resident directed culture, strengthen the ability of the household to grow as a separate effective home, and promotes individual and team learning in the household. The Household Nurse Leader provides each resident with professional licensed nursing assessments and evaluations and coordinates their care in conjunction with the Household Coordinator and other nursing staff in the household. It is imperative that the Household Nurse Leader and Household Coordinator have a solid working relationship in order to ensure the optimal functioning of the household.

DUTIES AND RESPONSIBILITIES OF THIS POSITION HOUSEHOLD NURSE LEADER:

  • Providing knowledge and skills to staff in all households in specific areas of clinical accountability, assuring that the households have the needed resources (community links, materials, supplies, skills and knowledge) to be successful in the Household Nurse Leader's professional area; grow people in multiple households to a professional expertise (specific areas of clinical accountability will be broad in scope when Household Nurse Leader is serving all households as clinical charge person, and narrow in scope when serving all households as mentor for specific clinical areas of accountability such as infection control, wound management or RAI leadership.)

  • Assuring the hiring, orientation, training and evaluation of all staff cross trained in the Household Nurse Leader's professional area is adequate to support quality outcomes in specific areas of clinical accountability.

  • Assuring regulatory compliance by encouraging appropriate record keeping and charting in all households and by maintaining appropriate files to document compliance in specific areas of clinical accountability.

  • Ensuring that all households participate in Quality Improvement measurements that will surface questions and issues to address, and that appropriate follow up is taken to assure quality outcomes, performing professional audits and observations to confirm the competency of all household guides and

household staff in the Household Nurse Leader's professional area specific areas of clinical accountability.

DUTIES AND RESPONSIBILITIES OF THIS POSITION

PRIMARY

  • Provides leadership to clinical staff from all households on specific areas of clinical accountability, specific accountability may include infection control, safety, wound management, falls, RAI process or other areas as determined by nurse council

  • Guides the implementation of clinical policies and procedures which ensure safe, sanitary and efficient practices supporting professional nursing care standard; assures that documentation, medication administration, resident nursing care, safety and sanitation, personnel and staff development, RAI process, materials management , CQI and resident rights are in compliance with facility standards

  • Guides household clinical staff to assure proper hiring, orientation, training, clinical competency, coaching, educating, counseling and performance appraisals are maintained in a manner supportive of team and professional standards of practice

  • Guides all household clinical staff to support organizational fiscal viability through efficient and effective use of all resources of the in the household, including maintaining clinical staffing and supply budgets within planned expenditures, maintain staffing patterns consistent with the philosophy, objectives and availability of resources

  • Guides the clinical component of continuous quality improvement in the household in support of a systemic approach to quality clinical care

  • In concert with the Household Coordinator, fosters collaboration, consensus and negotiation to address problem solving in the household, assures that all clinical policies and procedures of the household are maintained, interprets and communicates same to all in the household as needed, with an emphasis on resident, family or designated representative relationships

  • Maintains working relationship with physicians and other medical personnel in support of quality clinical care in the households

Willing to do any tasks needed, including those that may seem menial or unpleasant. When communicating with physician or other medical personnel advocates for resident preferences. Refrains from pressuring or manipulating residents into accepting medication or treatment for the sake of staff or facility convenience.

  • Critical Thinking: Recognizes early signs of changing conditions and uses all available and appropriate resources to resolve needs. Very strong observational and assessment skills with regards to physical and medical conditions, coupled with ability to discern effective response and treatment. Able to embrace individualized methods if they will best suit resident needs. Encourages all staff to bring areas of concern to attention and provides feedback regarding resolution.

  • Taking the Initiative: Sensitive to changing medical needs and the nature of staff responses, and takes appropriate actions regardless of whether the needs have been reported by others.

  • Learning and Teaching Centered: Is able to model person-centered practices, as well as evaluate and provide direction and coaching to other medical staff as needed.

  • Community and Team Builder: Able to facilitate sense of team connection and participation in concert with Household Coordinator. Able to nurture sense of accountability to residents and household among nursing staff. Able to advocate for needs of either household, resident, family or staff as needed.

  • Conflict Facilitation: Able to mediate conflicts concerning nursing staff as needed.

  • Interpersonal Sensitivity and Communication Skills: Able to provide clear directions and elicit trust and cooperation from staff. Good supervision skills - able to provide clear directions and expectations as well as providing clear, effective feedback. Able to demonstrate and elicit mutual trust and respect. Able to communicate resident's medical needs clearly with staff and families, and to facilitate other staff participation in medical care through eliciting others' observations, suggestions, etc. Able to communicate clearly and effectively with physicians.

  • Dementia Sensitivity: Able to teach and nurture care practices for persons with dementia that meet needs while protecting personhood and sense of well-being.

QUALIFICATIONS, KNOWLEDGE, SKILLS AND ABILITIES

  • Must meet regulatory requirements for licensure as a nurse in Kentucky. Certification in gerontological nursing is preferred, specialized training or experience in geriatric nursing is desirable. Three to five years on the job experience as nurse in skilled health care facility or retirement facility is preferred. Must have demonstrated leadership qualities, supervisory skills, interpersonal relationship skills, and knowledge of federal and state regulations governing clinical care, nursing practice and leadership. Commitment to person centered and resident directed care is essential. Certification in food handing (ServSafe or equivalent) is very strongly recommended.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

  • Will meet all physical requirements of the position as identified by the facility policies and procedures related to functional job analysis.

CONTINUING EDUCATION REQUIREMENTS

  • Will comply with all federal and state regulatory requirements to maintain licensure as a nurse, certification in food handling professional and other certifications as appropriate.

SUPERVISORY RESPONSIBILITIES: In collaboration with the Household Coordinator, the Nurse Leader will supervise all household staff members, conduct performance reviews on all household staff provide ongoing feedback to staff regarding their performance and be responsible for discipline of household staff based on facility policy and procedure. In partnership with the Household Coordinator and other household members, the Nurse Leader will participate in the hiring of household staff members.
