The Texas Health and Human Services Commission (HHSC) Medicaid CHIP Services (MCS) department seeks a highly qualified candidate to fill the position of Nurse II. MCS is driven by its mission to deliver quality, cost-effective services to Texans. This position makes a significant contribution to MCS's mission by conducting monitoring visits in Texas nursing facilities as identified by the Early Warning System (EWS). The nurse quality monitor utilizes expertise in the area of nursing to assess conditions that could be detrimental to the health, safety, and welfare of facility residents and assists providers in improving systems to ensure facilities are doing the right thing, in the right way, at the right time, for the right person to achieve the best possible outcomes.

Provides in-service education and other training to facility professional staff regarding evidence-based best practices.

Assists with the development and updating of program policy/procedures, evidence-based best practice standards and educational programs. Maintains collaborative relationships with their colleagues and team members. May train and supervise the work of others in the context of a quality monitoring team visit, orientation to peers and others as needed.

The nurse quality monitor reviews policy/procedures and performance reports to determine if the facility's systems are consistent with evidence-based practice. Documents accurate and complete reports of monitoring visits.

Communicates accurate information, findings, recommendations and technical assistance to the facility administrator and staff. Disseminates appropriate education tools and materials. Communicates pertinent information regarding visits with their supervisor. Attends work on a regular and predictable schedule and performs other duties as assigned.

Essential Job Functions:

Works collaboratively across MCS to identify innovative and effective solutions for clients and staff.

Conducts various types of visits including quality monitoring visits, rapid response team visits and in-service visits as assigned.

Monitors and evaluates facility systems through observations, interviews and record reviews identifying facility strengths and opportunities for improvement.

Provides technical assistance, in-services and other trainings to facility staff, families and others.

Conduct literature reviews and analyze complex information to update nursing related evidence based best practices technical assistance material.

Provides education on evidence-based best practices (EBBP).

Prepares complex visit reports and special narrative or statistical reports utilizing a computer.

Provides orientation and mentoring to peers and others as needed.

Works collaboratively with peers on assignments and special projects. Communicates on a complex level with others (internally or externally) to provide clinical information, answer inquiries, address issues and/or resolve problems.

Maintains current knowledge in assigned area by attending meetings, seminars or other trainings and by continually reviewing program policies, procedures and any changes.

Travels extensively each week with many overnight stays and attends work on a regular and predictable schedule in accordance with agency leave policy.

Performs other duties as assigned.

A minimum of two years of clinical RN experience with adults/geriatrics.

Knowledge of nursing facility, or other long-term care setting, rules, regulations, policies and/or operations.

Ability to write in a clear and concise manner, using correct grammar and punctuation.

Ability to teach and provide oral presentations.

Ability to work independently, exercise professional judgment and make sound decisions.

Ability to interpret and apply long term care facility regulations, standards of care in long term care and evaluate patient care.

Skill in the use of computers using a Windows-based operating system and Microsoft Office Suite.

Skill in organizing resources and establishing priorities.

Ability to operate and drive a vehicle.

Ability to perform other essential job functions such as sitting, standing, crawling, kneeling, bending and lifting up to 25 lbs.


Registration or Licensure Requirements:

This position requires a current Texas Registered Nurse license or by a state that recognizes reciprocity through the Nurse Licensure Compact (in good standing) and a current Texas driver's license. The applicant must enter their current RN license number and expiration date on the application. Initial Selection Criteria:

Current Texas Registered Nurse license (in good standing).

Current Texas driver's license.

Minimum of two years clinical RN experience with adults/geriatrics.

Willing to travel 80% of the time

Additional Information:

This is a mobile worker position requiring 80% region-wide and statewide travel including hours other than 8-5 and days other than Monday-Friday. Applicants must have reliable transportation.

PRINCIPAL DUTIES AND RESPONSIBILITIES: * Provide day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation. Participate in the department staffing and the appropriate hiring, firing and disciplinary actions. Recommend disciplinary action to Facility Administrator and initiate as appropriate. * Ensure compliance with all company and facility approved procedures and policies as well as regulations set forth by state and federal agencies. * Approve or disapprove time or personnel schedule changes in the absence on the Facility Administrator ensuring compliance with applicable regulations, policies and procedures for documenting time of work hours for staff members on assigned shift. * Participate in patient care plan meetings. * Maintain knowledge in the current practices related to the principles and techniques of dialysis by participating in all scheduled in- services. * Train and orient staff as necessary. * Routinely observe and guide direct patient care staff for appropriate technique and adherence to facility policies and procedures. * Promote and assist with compliance to OSHA programs in order to maintain a safe and clean working environment. * Maintain overall shift operation in a safe, efficient, and effective matter. * With Facility Administrator conduct staff meetings at least monthly or as needed to keep patient care staff informed of changes in patient care needs or operations to improve delivery of care. * Meet routinely with the Facility Administrator to discuss personnel and patient care status, issues, and information. * Supervise all documentation of patient information. * Coordinate Charge Nurse duties with Staff RNs acting in the relief charge capacity. * Assess daily patient care needs and develop and distribute patient care assignments appropriately. * Assume primary responsibility in an emergency situation. * Assess patient needs, respond to dialysis treatments, and communicate concerns to rounding physician. Implement changes in patient care/treatment as directed. * Monitor and supervise all patient care activity during dialysis and assist as necessary. * Collaborate with direct patient care team in making decisions to benefit patient care. * Continuously monitor patient's condition with regards to problems and potential complications associated with dialysis. * Administer medications to patients per physician's orders. * Act as the subject matter expert and as a resource for staff members. * Supervise and participate in completion of short and long term care plans. * Admit new patients according to facility procedure. * Ensure educational needs of patients are met and educate the patient and family about End Stage Renal Disease, dialysis therapy, diet and medication. * Supervise the safe and effective use of all equipment involved in direct patient care. * Operate all dialysis related and emergency equipment safely and efficiently when needed. * Perform required testing and verification and initial the checklist for start-up and shut-down procedures as outlined in the Technical Services Manual. * Complete Nurse's Technical Training Program/Water Quality Facility * Assist with special projects or other duties as assigned by the Facility Administrator * Assist with the interviewing of potential direct patient care staff * Promote efficient use of medical supplies. * Attend and participate in monthly Quality Assurance meetings.
