Rising has an opening for a full-time Nurse Case Manager located in Springfield, IL! This is a dedicated nurse case manager role, providing case management to one of our local clients. Our ideal candidate is experienced in workers compensation and has experience with early intervention and a wide variety of cases. This is a hybrid role, with some flexibility working onsite with a client and working from home.

What exactly does a Nurse Case Manager do?

  • Assesses and analyzes injured worker's medical and/or vocational status and develops a viable, realistic action plan for maximum medical improvement from date of referral to closure.
  • Meets with individuals either telephonically or in their homes, work-sites, physician's or therapist's office to provide ongoing case management services.
  • Assesses the assigned individual to ensure appropriate safety measures are in place.
  • Implements, calculates and documents cost savings measures.
  • Manages progress towards desired outcomes.
  • Responsible for maintaining regular communication with the injured worker, medical providers, attorneys, employers, claims professionals, and Rising management.
  • Develops guidelines for working with patients,healthcare management companies, insurance companies or other third party payers to determine quality, cost-effective care.
  • Applies all special service instructions required by customers/accounts.
  • Appropriately applies all laws and regulations to the provision of rehabilitation services.
  • As needed, testifies to substantiate any casework or reports that may be deemed relevant in litigated cases.
  • Evaluates outcomes and case goals.
  • Prepares reports and other required documentation for casework activities.
  • Arranges referrals, consultations, therapeutic services, and confers with other specialists regarding course of care and treatment.
  • Upholds applicable state and federal regulatory requirements that govern workers' compensation disability.
  • Substantiates case activities carried out/services rendered in all activity reports completed.
  • Accurately reports all case activity and billable hours in accordance with client-specific guidelines and consistent with company billing practices.
  • Attends all scheduled conference calls as mandated by management.
  • Maintains any required credentials and adheres to all codes of ethics required by these credentials.
  • Performs various professional duties as assigned by management.


  • RN with an active license in Illinois
  • Minimum 2 years clinical nursing background.
  • 5 years case manager experience preferred

Responsibilities Summary: Provide supportive services in Springfield using the Pathway of Hope strengths-based case management approach to individuals and families with a desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives. Pathway of Hope - Essential Functions * Conduct outreach each night in designated homeless areas engagement services in Springfield in order to identify and support participants for Pathway of Hope case management services. Engage and build rapport with the target population and meet the desired outcomes of eligibility requirements. * Collaborate with local leadership to best support clients and their designated goals. * Collaborate with other local agencies and the police department to connect clients to services. * Maintain an appropriate caseload (at least 10 unduplicated families per fiscal year and carry at least 5-7 active families at any given time). Provide appropriate referrals and linkage to support individuals and families of the local community. * Perform client intake including any assessments and measurements and ongoing follow up needs as appropriate. Must be able to assess any serious personal safety and mental health issues. Provide information on available community resources and referrals as necessary. * Maintain an accurate and updated case plan to support stability and self-sufficiency to include both short and long-term goals with related activities and action steps. Keep emergency contact information for each homeless client. * Maintain written and electronic case files with all relevant client information along with advocacy and financial services as necessary. (i.e., court appointments, landlord/tenant conflict resolution, etc.) * Engage and collaborate by participating in monthly supervision, divisional case manager cluster call-ins, and local Pathway of Hope team meetings driven by the POH Regional Coordinator to ensure best practices. * Create and maintain electronic comprehensive demographic data as required on all participants. Must be able to enter in all data required into the Salvation Army Information Management System promptly and with a high level of accuracy. * All other duties as related to Pathway of Hope Case Management as assigned by Corps Officer Qualifications Qualifications: Education: Bachelor's degree in the human service area or significant experience will be considered in lieu of. Experience: A minimum of two years of progressive years of case management and client support in the social services profession or related educational experiences. Skills: Individual must be able to work in a confidential manner and manage the varied demands of working with vulnerable, high-risk individuals and families. Having the ability to work effectively with a multicultural or varying socioeconomic status caseload. Individuals must be flexible in order to handle unexpected crises with calmness and good judgment; must have a working knowledge of community resources, good communication, and organizational skills, effective problem-solving skills and are able to relate well to adults in a patient, courteous and respectful manner. Must have knowledge of the computer particularly Microsoft products and social media platforms. All employees must be able to pass a background check and MVR for driving TSA Vehicles.
