Rising has an opening for a full-time Nurse Case Manager located in Springfield, IL! This is a dedicated nurse case manager role, providing case management to one of our local clients. Our ideal candidate is experienced in workers compensation and has experience with early intervention and a wide variety of cases. This is a hybrid role, with some flexibility working onsite with a client and working from home.

What exactly does a Nurse Case Manager do?

  • Assesses and analyzes injured worker's medical and/or vocational status and develops a viable, realistic action plan for maximum medical improvement from date of referral to closure.
  • Meets with individuals either telephonically or in their homes, work-sites, physician's or therapist's office to provide ongoing case management services.
  • Assesses the assigned individual to ensure appropriate safety measures are in place.
  • Implements, calculates and documents cost savings measures.
  • Manages progress towards desired outcomes.
  • Responsible for maintaining regular communication with the injured worker, medical providers, attorneys, employers, claims professionals, and Rising management.
  • Develops guidelines for working with patients,healthcare management companies, insurance companies or other third party payers to determine quality, cost-effective care.
  • Applies all special service instructions required by customers/accounts.
  • Appropriately applies all laws and regulations to the provision of rehabilitation services.
  • As needed, testifies to substantiate any casework or reports that may be deemed relevant in litigated cases.
  • Evaluates outcomes and case goals.
  • Prepares reports and other required documentation for casework activities.
  • Arranges referrals, consultations, therapeutic services, and confers with other specialists regarding course of care and treatment.
  • Upholds applicable state and federal regulatory requirements that govern workers' compensation disability.
  • Substantiates case activities carried out/services rendered in all activity reports completed.
  • Accurately reports all case activity and billable hours in accordance with client-specific guidelines and consistent with company billing practices.
  • Attends all scheduled conference calls as mandated by management.
  • Maintains any required credentials and adheres to all codes of ethics required by these credentials.
  • Performs various professional duties as assigned by management.

Requirements

  • RN with an active license in Illinois
  • Minimum 2 years clinical nursing background.
  • 5 years case manager experience preferred

This position is responsible for bringing services, agencies, resources and other people together to assist a client in achieving established goals. Essential Duties and Responsibilities * Provide direct services and coordinate other services, resources and people to assist clients in achieving their established service plan goals * Screen and assess clients for their appropriateness for agency services * Adheres to the service hour protocol established by the agency * Support engagement in drug treatment, community, etc. * Counsel and aid family members to assist them in understanding, dealing with, and supporting the client or patient * Facilitate individual and/or group orientation or educational sessions (as required) * Facilitate client drug test (as required) * Represent clients and agency at staffing and court hearings, probation, parole and provider networks * Provide referrals for identified needs of the client and crisis intervention when needed * Document the results of assessments, service plans, reports, progress notes, discharge summaries and other client-related activities including accurate reporting of services * Attend in-service trainings, conferences and education sessions (as required) * Complete and maintain all required documentation pertaining to clients (documentation, ECR, letters and status reports) Qualifications * Bachelors degree preferred in human services, social work, psychology or other related fields * One or more years experience in human service activities; or any equivalent combination of training and experience * Knowledge of human behavior for the assessment and signs and symptoms of substance abuse disorders. Specific knowledge necessary for working with special populations * Knowledge of the Code of Ethics for substance abuse professionals * Knowledge of the federal drug and alcohol confidentiality law (42 C.F.R. Part 2) and the Health Insurance Portability and Accountability Act * Ability to obtain CARS or CADC certification through the State Certification Board (IAODAPCA) within the first two years in the field (if required by program). * Knowledge of the criminal justice system * Knowledge of computers, including proficiency in Microsoft Office applications and the Internet * Posses clinical skills to effectively manage clients * Ability to work with a diverse population * Ability to work flexible hours and travel as needed (evening/weekend hours) * Must be available to attend meetings and meet the needs of the program and/or clients throughout the city, counties or wherever needed (a valid drivers license, current auto insurance and reliable automotive transportation are required) Physical Demands The physical demands described here are representative of those that must be met by an employee to perform the functions of this job.
