Nurse Case Manager

Nurse Case Manager Work with a specialized population, in their homes. Review records, assess health and wellness of the Individual . Provide recommendations for improved health and wellness outcomes.

Educate, demonstrate, facilitate the supports staff in their efforts to manage the health care components of the Individual they support Use of personal vehicle required. Gas stipend provided. Flexible schedule . Days, evenings or weekends.

Work when you prefer. Documentation is on electronic device (provided). Narrative format. Must be able to consistently meet deadlines for documentation, reporting of dates/times of service.

Good written and oral communication skills required. Must be able to self monitor, and represent themselves professionally, while being relatable to the Individual and support staff. Crossley Nurse Consultants is a small privately owned company.

We provide Case management and skilled services to adults with Intellectual and/or developmental disabilities, living in residential community settings. We have provided services in the Las Vegas area for over 30 years We enjoy a close working relationship with our clients and their support staff. We prioritize the clients individual needs and desires for their best life. We enjoy the ability to manage our own schedules and perform our duties independently .
