Location: Any Anthem Location

Hours: Four days a week 8:30am

  • 5:00pm and one day 11:30am

  • 8:00pm

Responsible for performing care management within the scope of licensure for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum. Performs duties telephonically or on-site such as at hospitals for discharge planning. Primary duties may include, but are not limited to:

  • Ensures member access to services appropriate to their health needs.

  • Conducts assessments to identify individual needs and a specific care management plan to address objectives and goals as identified during assessment.

  • Implements care plan by facilitating authorizations/referrals as appropriate within benefits structure or through extra-contractual arrangements.

  • Coordinates internal and external resources to meet identified needs.

  • Monitors and evaluates effectiveness of the care management plan and modifies as necessary.

  • Interfaces with Medical Directors and Physician Advisors on the development of care management treatment plans.

  • Negotiates rates of reimbursement, as applicable.

  • Assists in problem solving with providers, claims or service issues.

NURSE CASE MANAGER I:

Requires a BA/BS in a health related field; 3 years of clinical experience; or any combination of education and experience, which would provide an equivalent background. Current, unrestricted RN license in applicable state(s) required. Multi-state licensure is required if this individual is providing services in multiple states. Certification as a Case Manager is preferred.

NURSE CASE MANAGER II:

Requires a BA/BS in a health related field; 5 years of clinical experience; or any combination of education and experience, which would provide an equivalent background. Current, unrestricted RN license in applicable state(s) required. Multi-state licensure is required if this individual is providing services in multiple states. Certification as a Case Manager is preferred. Oncology or Diabetes nurse certification is preferred

  • May be hired as a Nurse Case Manager I or II depending upon level of education and relevant experience.*

Position: Nurse Disease Management I, II or Senior The hiring manager will determine level of hire based upon the selected applicant's skillset relative to the qualifications listed for the position. Location: This position can be located in Atlanta, GA; St. Louis, MO; Indianapolis, IN; Richmond, VA; or Woodland Hills, CA. Shift: An 8 hour shift Monday – Friday between 11:00 am – 11:00 pm est. The Nurse Disease Management will be responsible for outreach, enrollment and management of Disease Management members. Provides education and coordination to address gaps in care for multiple managed conditions to ensure cost effective and efficient utilization of health services. Conducts assessment, planning, implementation, coordination, monitoring and evaluation of members. Nurse Disease Management I Responsible for participating in delivery of patient education and disease management interventions and for performing health coaching for members, across multiple lines, for health improvement/management programs for chronic diseases. Primary duties may include, but are not limited to: * Conducts behavioral or clinical assessments to identify individual member knowledge, skills and behavioral needs. Identifies and/or coordinates specific health coaching plan needs to address objectives and goals identified during assessments. * Interfaces with provider and other health professionals to coordinate health coaching plan for the member. * Implements and/or coordinates coaching and/or care plans by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral health needs. * Uses motivational interviewing to facilitate health behavior change. * Monitors and evaluates effectiveness of interventions and/or health coaching plans and modifies as needed. * Directs members to facilities, community agencies and appropriate provider/network. Nurse Disease Management II Responsible for participating in and delivery of more complex patient education and disease management interventions and performs health coaching for members, across multiple lines, for health improvement/management programs for chronic diseases. Primary duties may include, but are not limited to: * Conducts behavioral or clinical assessments to identify individual member knowledge, skills and behavioral needs. * Identifies and/or coordinates specific health coaching plan needs to address objectives and goals identified during assessments. * Interfaces with provider and other health professionals to coordinate health coaching plan for the member. * Implements and/or coordinates coaching and/or care plans by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral health needs. * Uses motivational interviewing to facilitate health behavior change. * Monitors and evaluates effectiveness of interventions and/or health coaching plans and modifies. * Directs members to facilities, community agencies and appropriate provider/network. * Serves as a resource to lower-leveled DM associates. * Refers member to catastrophic case management as appropriate. Nurse Disease Management Senior Responsible for providing clinical expertise in design and delivery of the most complex patient education and disease management interventions and performs health coaching for members, across most lines, for health improvement/management programs for chronic diseases. Primary duties may include, but are not limited to: * Conducts behavioral or clinical assessments to identify individual member knowledge, skills and behavioral needs. * Identifies and/or coordinates specific health coaching plan needs to address objectives and goals identified during assessments. Interfaces with provider and other health professionals to coordinate health coaching plan for the member. * Implements and/or coordinates coaching and/or care plans by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral health needs. Uses motivational interviewing to facilitate health behavior change. * Monitors and evaluates effectiveness of interventions and/or health coaching plans and modifies. * Directs members to facilities, community agencies and appropriate provider/network. Refers member to catastrophic case management as appropriate. * Assists with development of new programs and initiatives, leads departmental projects or participates on cross-functional teams.
