The Nurse Case Manager position is responsible for performing care management within the scope of licensure for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum. Performs duties telephonically or on-site such as at hospitals for discharge planning.

Primary duties may include, but are not limited to:

  • Ensures member access to services appropriate to their health needs.

  • Conducts assessments to identify individual needs and a specific care management plan to address objectives and goals as identified during assessment.

  • Implements care plan by facilitating authorizations/referrals as appropriate within benefits structure or through extra-contractual arrangements.

  • Coordinates internal and external resources to meet identified needs.

  • Monitors and evaluates effectiveness of the care management plan and modifies as necessary.

  • Interfaces with Medical Directors and Physician Advisors on the development of care management treatment plans.

  • Negotiates rates of reimbursement, as applicable.

  • Assists in problem solving with providers, claims or service issues.

  • Requires a Bachelor's degree in a health related field.

  • 3 or more years of clinical experience; or any combination of education and experience, which would provide an equivalent background.

  • Current, active, unrestricted RN license in the state of Florida is required.

  • Medicaid and/or managed care experience preferred but not required.

  • Ability to travel required (day trips with all business related expenses reimbursed).

Responsibilities The Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures. Required Qualifications * Licensed Registered Nurse (RN) in the stat of Florida with no disciplinary action * 3-5 years of clinical acute care experience * Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook * Reside in South Florida * Willing to work 2nd Shift: Monday-Friday 10:30am to 7:00pm or 11:00 AM to 8:00 PM Eastern Time * Monthly onsite visits at facilities and as business needs. Additional Requirements: * Must have a separate room for privacy with a locked door during working hours and during communication with members. * Must provide a high speed DSL or cable modem for a home office (Satellite and Wireless Internet service are prohibited). A minimum standard speed of10x1 (10mbs download x 1mbs upload) for optimal performance of is required. * Valid driver's license, car insurance, and access to an automobile * This role is a part of Humana's Driver Safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits. * This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program.
