Nurse Case Manager

An exciting opportunity to work for a hospital in Delaware as their Nurse Case Manager is now available ] The Nurse Case Manager will work for a non-profit health system, known for their acute care hospital, walk-in centers,, and speciality services. The highly accredited 200 bed acute care hospital received The Joint Commission’s Gold Seal of Approval for their Joint Replacement, Stroke, and Heart Failure programs.

The Nurse Case Manager will work in an acute care setting and provide care coordination services to patients through cross collaborating with multidisciplinary teams, evaluating options and developing patient discharge options that meet an individual's need. The Nurse Case Manager will join the Care Coordination team, working alongside a team of specialized case managers holding their CCM Commission for Case Manager Certification. Ideally the Nurse Case Manager will have previous Case Management experience working with an inpatient population within an acute care setting.

Additionally, this individual will be up to date on best practices, state, federal and hospital regulatory compliances and guidelines, have prior discharge planning assessment and utilization management experience, and hold an active registered nurse license. The Nurse Case Manager will work at a hospital just off the coast of Delaware.
