Job Description Job Description A-Line Staffing is now hiring a Registered Nurse (RN) - Case Manager in Cincinnati, OH If you are interested in this Registered Nurse (RN) - Case Manager position, please contact Marcia Hamilton at (586) 710-7979 or mhamiltonalinestaffing.com Registered Nurse (RN) - Case Manager Compensation The pay for this position is $ 44.14 per hour Benefits are available to full-time employees after 90 days of employment A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Registered Nurse (RN) - Case Manager Highlights This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs The required availability for this position is FULLY REMOTE AND TRAVELING TO SEE PATIENTS Monday - Friday 8:00 am – 5:00 pm Registered Nurse (RN) - Case Manager Responsibilities · Assess member health needs and benefit eligibility through comprehensive reviews of clinical data and member information. · Develop individualized care plans, ensuring appropriate services and resources are in place for optimal outcomes. · Coordinate care and services for members via phone or face-to-face meetings, helping to resolve issues and facilitate transitions into programs. · Advocate for members by providing them with the support needed to navigate their health journey, ensuring their needs are met through collaboration with healthcare providers and internal/external resources. · Implement case management processes that adhere to regulatory and accreditation standards. · Apply clinical judgment to address complex health and social factors impacting the member’s health and wellness. · Consult with supervisors and multidisciplinary teams to overcome care barriers and ensure optimal member outcomes. Registered Nurse (RN) - Case Manager Requirements Ohio License RN Associate's Degree or higher (RN with current unrestricted state licensure) Case manager experience 2 years of clinical nursing experience Attendance is mandatory for the first 90 days Graduation from high school or equivalent.
