JOB DESCRIPTION:

This role will be responsible for managing the review of clinical documentation and assists in the training of clinical staff to ensure accuracy of OASIS and reimbursement, regulatory compliance and the delivery of outcome-based, cost effective care. This role will collaborate with all team members to achieve exceptional clinical, quality and financial outcomes.

JOB RESPONSIBILITIES

Reviews clinical information and assessment data and makes recommendations for efficient and effective utilization of services to achieve positive patient outcomes and established organizational metrics

Collaborates with clinicians and peers across regions to ensure accuracy of data using the patients electronic health record as well as a software assisted record review to ensure OASIS accuracy and reliability while monitoring timeliness of clinician response

Teach, coach and mentor staff in making clinical and utilization management decisions that are cost effective and focused on clinical outcomes and operational performance indicators

Identifies OASIS accuracy trends for clinicians and teams based on record review and partners with clinicians and Managers to study and adjust processes and standards to achieve desired outcomes

Plan of care and supplemental orders review and other duties as needed

Education:

RN

Minimum Experience
1.Minimum of 3 years planning, scheduling or like function
2.Minimum of 3 years planning, scheduling or like function within Homecare operations setting
3.Familiarity with Homecare regulation and OASIS
4.Computer literacy
5.Lean or Six Sigma Experience

Preferred Experience
1.OASIS and Chronic Care Management certification preferred
2.Minimum of 5 years planning, scheduling or like function within Homecare operations setting
3.Familiarity with Homecare regulation and OASIS
4.Computer literacy and familiarity with SHP and other software assisted record review tools preferred
5.Lean or Six Sigma experience
