Nurse Care Coordinator

Responsibilities

The Nurse Care Coordinator serves in an expanded nursing role to collaborate with patients and their primary care providers to offer a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. The Nurse Care Coordinator:

• Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status.

• Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the populations.

• Develops systems of care that monitor patient progress and promote early intervention in acute care situations.

• Works effectively with other members of the health care team to optimize interventions.

• Manages utilization and practice metrics to further refine the delivery of care model to maximize clinical, quality, and fiscal outcomes for identified populations.

Qualifications

Education and Experience:Associate's Degree, Diploma, or Certification in Nursing and over 6 months to 1 year office RN experience required BSN preferred Licenses and Certifications:Basic Life Support (BLS), Upon hire, required

Registered Nurse in PA - Licensed, Upon hire, required
