We have an amazing opportunity for a Nurse Admissions Liaison that can:
Meets with potential residents/patients, localities, and families to educate them on the facility services offered.
Develops strategies to maximize admissions of residents in accordance with facility plan.
Ensures facility is able to provide appropriate level of clinical care.
Travels to hospitals to assess acute care residents; may also be required to do home-site visits.
Maintains contact with and coordinates care with patients who return to hospital from facility to ensure facility care was satisfactory.
Coordinates pre-authorization for managed care admissions.
Evaluates effectiveness and cost benefit of marketing tactics. Makes recommendations to alter strategies.
Reviews census management practice guidelines, analyzes trends, and takes action to avert deterioration of market share.
Develops marketing tools for facility within budgetary constraints.
Responsible for relationship development and management of health partners and organizations; sources new business.
Represents the company with professionalism to community and referral sources (i.e., case managers, discharge planners, social workers and physicians).
Develops and maintains positive relationships to gain referrals.
Maintains a strong working relationship with attending physicians in area hospitals to promote skilled services, assists with knowledge of geriatric medications and assists with drug interchanges.
Coordinates external marketing of facilities within the community through health providers and organizations.
Verifies financial resources of applicant.
Completes required forms and documents in accordance with company policy and state and/or federal regulations.
Works with local community agencies to generate a positive image and encourage word-of-mouth referrals.
Plans and oversees the execution of community based promotional events, open houses, and educational seminars.
Communicates with local media sources to ensure community awareness of facilities services.
Participates in company trade shows, health fairs, and marketing activities as assigned.Assumes 24-hour responsibility and accountability for resident care.
Communicates with the Purchasing Manager the need for any special equipment (ex. CPM).
Notifies the Director of Nursing and/or the Case Management department of any potential risks associated with the admission.
Receives all necessary documentation from the referring institution.
Obtains physician discharge orders and/or the medication reconciliation form completed by the discharging physician from the transferring facility.
Completes the admission orders with the assigned attending physician in conjunction with the medication reconciliation form and faxes the orders to the pharmacy.
Complete other duties as assigned
Licensed RN with experience in marketing or a related field is preferred.
One to three (1-3) years of experience in health care, long-term care industry. Licensed RN must have a minimum of one (1) year experience in patient/resident assessments.
Maintains an appropriate level of knowledge about sources of payment.
Must be able to travel 75% of the time on day trips to multiple hospitals in the area.
Able to work flexible schedule and some weekends required.
Knowledge and experience related to the physical and emotional needs of the elderly, disabled.
Knowledge of the laws, regulations, and guidelines that pertain to long-term care, including Medicare, Medicaid.
Must understand the admissions process from acute care to long-term care.
Public relations experience preferred.
