Description of Work

Position Definition: A registered professional nurse who performs general public health nursing and related community activities and carries increased responsibilities for clinical practice in public health nursing.

General Duties:Supervises individuals and families in the prevention of disease, promotion of health and rehabilitation of the ill. Performs mandated public health services.

Interviews and instructs diagnosed and contact cases of communicable diseases to promote cure and prevent contagion. Participates in health department clinical services.

Interviews clinic patients.

Interprets to patients and their families the psychosocial and pathophysiological aspects of their illness. Keeps records and prepares reports. Assists in promoting the development of public health programs in the community.

Participates in the orientation of new nursing staff. Participates in coordinating the planning for staff education programs.

Coordinates, monitors and evaluates public health activities and programs.

Collaborates with other Health Department programs, Connecticut Department of Public Health and local community agencies. Provides educational programs and consultation to professional groups, community organizations and the general public utilizing specialized knowledge and expertise. Develops and implements liaison networks with other health professionals.

Represents the Department of Health at community meetings and professional organizations. Maintains accurate and appropriate records and statistical data for reporting, analysis and trend identification. Participates in quality assurance and improvement activities.

Additional Duties: Uses extensively the associated services available to the Health Department and initiates referrals to related health and social agencies. Participates in student education programs.

Supervised By: Receives general direction from the Supervisor of Clinical Services.

Required Knowledge, Skills and Abilities

Knowledge of public health principles and practices. Knowledge of theory and processes of the physical, social, biological, and behavioral sciences and their applications to nursing science.

Knowledge of concepts and processes related to communities, including epidemiology and biostatics, risk factors and implications for selected populations, resource assessment techniques, environmental factors and social organization. Knowledge of the principles and practice of teaching-learning processes, change processes, family dynamics, and cultural diversity. Ability to use electronic medical records software and software for word processing, spreadsheets, and database management. Ability to work with members of the community.

Minimum Qualifications

Graduation froman accredited baccalaureate nursing program and one year of public health experience or equivalent combination of relevant education, training and experience as described above.

Special Licenses/Certifications AND/OR Supplemental Information

License or Certificate: Possession of a valid license as Registered Nurse in State of Connecticut. Must have a valid driver's license and reliable insured transportation.

Hours:Up to 19 hours per week
