Inpatient Oncology Consult Advanced Practice Provider- NWH

MGH Cancer Center at Newton Wellesley Hospital is looking for an Advanced Practice Clinician to join the Inpatient Hematology/Oncology Hospitalist physicians at Newton Wellesley Hospital.

The team is responsible for caring for the inpatient hematology/oncology patients in a consultant role. The NP/PA will be a collaborative team member and will help to assist in providing continuity of patient care and optimal communication between the inpatient and outpatient settings. The focus of the position will be the clinical care of solid tumor patients at Newton Wesley Hospital, although coverage of some hematology will be required. Emphasis is placed on building clinical relationships with other caregivers at NWH, MGH, and in the community. Full Time, Monday through Friday.

Provides consultative care to the Heme-Onc inpatients in collaboration with physician(s) and nursing staff.

Performs and records the patient history and physical exam.

Documents daily observations and interventions according to established standards and round daily to provide guidance and facilitate care plans.

Orders and interprets appropriate laboratory, radiological and other diagnostic studies, CT scans, MRIs and any invasive diagnostic testing deemed appropriate in consultation with supervising and/or attending physician.

Facilitates referrals from and to other specialists/therapists, as indicated. Identifies, prevents or solves complex and /or recurring patient care problems.

Follows and manages patients while inpatient to enhance continuity of care and assist with admissions and discharges.

Performs (oversees/assures completion of) specialized procedures particular to the select population mutually agreed upon with collaborating and/or attending physician(s).

Writes admitting/discharge orders and completes discharge summaries ensuring appropriate communication of discharge plans to responsible clinicians and home service agencies.

Performs medically delegated aspects of patient care as agreed upon by medical administrators according to practice guidelines.

Writes orders which are in accord with current guidelines and maintains prescriptive-writing authority.

Obtains consents for procedures, chemotherapy and blood transfusions as well as other disease specific procedures.

Provides consultation to both nurses and physicians as well as provide guidance and educational programming for inpatient nursing staff.

Participates and attends intradepartmental meetings, Team Conferences Identifies system-wide factors contributing to undesirable outcomes, such as complications, increased length of stay and over-utilization of services.

Facilitates patient movement through the system and course of treatment.

Provides efficient use of resources through interdepartmental collaboration.

Responsible for own professional growth and maintenance of required credentials.

Maintains and updates clinical knowledge and skills based on current oncology practice/research.

  • Oncology specific experience preferred.

  • Demonstrated competence in specialized area.

  • Ability to effectively communicate clinical information, care plans, etc. to medical and lay audiences; able to educate patients and their families regarding routine and complex medical information.

  • Strong organizational and time management skills.

  • Ability to work independently as well as a member of a team; can effectively influence and lead as necessary.

  • Effectively uses analytical skills and problem-solving techniques in both clinical and operational matters.

  • Current licensure in the commonwealth of Massachusetts as a registered NP or PA

  • Master's Degree in Nursing or completion of NCCPA program

  • Current certification as a Nurse Practitioner or Physician's Assistant in appropriate specialty to care for adult patients.
    2 Years Advanced Practice experience preferred, Acute Care Experience preferred.

These include, but are not limited to, direct patient care activities such as taking vital signs and assisting patients with the basic activities of daily living, as well as non-clinical activities such as acting as a receptionist, coordinating equipment, providing transportation activities, and performing tasks specific to the department assigned. The Patient Care Assistant II interacts with patients, families, nursing, and a variety of interdisciplinary personnel and possesses strong interpersonal and communication skills. The PCA II staff collaborate amongst themselves to ensure safe patient care and smooth unit operations. Essential Functions: * Demonstrates NWH CAREfirst standards of behavior while working toward accomplishing the hospital goals: acknowledges customer, introduces self, explains situation, and answers questions in a warm and welcoming manner * Prepares patient rooms for admission * Orients the patient and family to the unit/room upon admission * Keeps the patient and family informed of scheduled routines and activities * Ensures patient safety and promotes comfort * Assists patients with activities of daily living such as hygiene, elimination needs, ambulation, and nourishment; performs these activities for patients who are unable to care for themselves * Takes vital signs; performs blood sugar testing; measures patient's height and weight; records intake, output and amount eaten. Reports significant changes to the Registered Nurse * Documents patient data in the medical record in a timely manner – dates and times all entries * Ensures a safe environment for staff and specific populations served * Maintains appropriate situational safety standards, such as personal protective equipment and proper body mechanics during patient care activities * Maintains annual job requirements/competencies such as mandatory education, CPR, and employee health requirements * Performs duties as assigned under the direction of the RN * Receives and reports patient information to the RN * Contributes to the plan of care through communication with the Registered Nurse * Observes and reports changes in patients' condition to RN * Runs errands as directed * Uses the Premise System to request transport for the purpose of picking up supplies and equipment * Transports patients via stretcher, wheelchair, bed, isolette, crib, or escorts to various areas within the Hospital * Transports equipment, supplies, patient belongings, pharmaceuticals and specimens to and from other departments, as requested * Ensures the CAREfirst standards of service excellence are consistently used to enhance the overall patient experience * Provides 1:1 patient observation as assigned * Maintains environment, supplies and equipment * Orders supplies and equipment as directed by the manager or charge nurse * Provides effective handoff to relieving Patient Care Assistant and reports off to assigned nurse prior to break and at end of assigned shift * Performs phlebotomy procedures * Performs secretarial duties as needed Job Knowledge and Skills: * Successful completion of the Patient Care Assistant Proficiency Exam with a score of 80% or better (experienced PCAs and Nursing students) * Successful completion of the literacy exam with a score of 80% or better (all candidates) * CPR training required * Phlebotomy training preferred * Must be able to speak and understand written and oral English communication Education: * High school graduate or equivalent Experience: * Six months of experience as a Nursing Assistant, or is a Nursing student currently enrolled in a formal program who has successfully completed at least one clinical rotation, or is currently enrolled in and successfully completes the NWH Patient Care Assistant Training Program. * Phlebotomy experience preferred Partners Healthcare System Newton MA
