General Description

Christiana Care Health System is looking for a motivated full-time Nurse Practitioner or Physician Assistant to work in support and work in collaboration with our Christiana Care Hospitalist Partners (CCHP) on our Hematology/Oncology Unit. The chosen candidate will work closely with the attendings in the Hematology/Oncology group to provide direct patient care for all Hematology/Oncology patients on the 6E unit.

Christiana Care Hospitalist Partners is a forward thinking Hospitalist practice emphasizing continuity of care, patient satisfaction and quality. Our Hospitalists, in partnership with primary care physicians, provide the highest quality of patient care in directing, treating and coordinating all aspects of the patient's hospitalization.

Principal Duties and Responsibilities

Provides clinical support to Christiana Care Hospitalist Partners (CCHP) in inpatient and outpatient settings. Adheres to all regulatory and organizational nursing policies and procedures to deliver excellent patient care. Works collaboratively with the clinical team, social workers, pharmacists, family members and others to ensure safe and seamless transitions of care.

Clinical Duties

Visits newly admitted patients in a timely fashion per department policy.

Upon request of CCHP/Hematology/Oncology physician, performs consultations on inpatients.

Upon request of CCHP/Oncology/Hematology/Oncology physician, collaborates with that physician, subspecialists, RNs and other members of the health care team to provide direct patient care.

Establishes priorities based on patient needs and act on these accordingly.

Individualizes medical care plan and transition of care to outpatient setting based on patient assessment.

Provides appropriate patient education and counseling to patient and families in ways understandable to them.

Provides excellent customer service and assists in efforts to improve Christiana Care Hospitalist Partners' patient satisfaction scores.

Administrative Duties

Attends all scheduled meetings to include monthly hospital medicine meetings and performs all tasks as assigned.

Assists in orientation of new hospitalists and nurse practitioners

Actively participates in administrative/citizenship duties which may include but is not limited to: organizational and planning meetings, committee memberships and educational conferences.

Education and Experience Requirements

Licensed or eligible for Nurse Practitioner or Physician Assistant license in the state of Delaware

Certification in specialty area

Prescriptive Authority or eligibility for the state of Delaware

DEA registration required

BLS & ACLS are required

Christiana Care Cardiology Consultants is a large practice consisting of more than 20 physicians and 16 advanced practice clinicians providing specialized cardiac care for residents in DE, PA, NJ and MD. This position covers inpatient areas at Christiana & Wilmington Hospitals, and various outpatient locations. Job requires rotating through a variety of roles which include but are not limited to daily rounding, ED and floor consults, pre and post procedural care, stress test monitoring, and both acute and chronic outpatient management of patients with a cardiology diagnosis. The primary shift is comprised of 10 hour days from 7am – 5pm with additional night, weekend and holiday requirements. PRINCIPAL DUTIES AND RESPONSIBILITIES: Facilitates care through all phases of patient illnesses Reviews and retrieves data pertinent to patient's medical history. Performs and records physical assessment in response to consult and collaborates with physicians and other members of the medical staff. Screens patient to determine need for urgent medical attention. Evaluates need to changes in patient therapy. Writes orders and modifies treatment plans. Prescribes medication with credentials provided by the Joint Practice Committee of the Delaware Board of Medical Practice. Provides education and instruction to patients regarding care. Organizes appropriate discharge plans. Assesses medical device surgical sites including dressing changes, staple removal or suture removal. Actively participates in other aspects of Cardiology, by performing procedures that replace physicians: special oriented care such as monitoring stress tests and implantable device center. Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors. Performs other related duties as required. SPECIAL REQUIREMENTS: Licensure as Registered Nurse and Nurse Practitioner as set forth by the Delaware Board of Nursing; or as a Physician's Assistant as set forth by the Delaware Board of Medical Practice Prescriptive authority for non-controlled substances in the State of Delaware; Controlled Substance Registration for the State of Delaware and Commonwealth of Pennsylvania; Federal DEA Registration. CPR certification in Health Care Provider; ACLS certification Prior cardiology and/or ICU experience preferred; new graduates are welcome to apply For NP's: Acute Care certification preferred.
