PRACTICE SPECIFICS:

This is a team-based patient centered care in a rural clinic, high clinical quality, community health focus. The department team is made up of 7 Physicians, 3 APRN/PA's and 15 additional staff:

CA's, RN's - additional support departments including: Lab, Radiology, Physical Therapy, and Pharmacy.

Effectively manage chronic disease including clinical management, relationship building, patient education, activation and engagement as part of the primary care team.

Utilizing advanced health assessment, knowledge, and decision making skills, the Nurse Practitioner or Physician Assistant works independently and in collaboration with physicians and other health care professionals.

Position requires high level of customer service skills to establish and enhance positive relationships with patients, co-workers, and others.

Desired Skill set/Scope of Practice: Family Medicine Focus, Joint Injections, Sport Medicine, Dermatology, and Tobacco Cessation Counseling.

Estimated Patients seen a day: 16-21 Patients

Licensure/Credentials

1.Current licensure as a registered nurse in appropriate state

2.Current license as a Nurse Practitioner or Physician Assistant in appropriate state

3.Current certification in CPR

4.National certification in applicable area

5.Successful completion of Essentia Health credentialing process prior to practice

NP:

Current licensure as a registered nurse in appropriate state.

Current licensure as a Nurse Practitioner in appropriate state.

Current certification in CPR.

National certification in applicable area.

Successful completion of Essentia Health credentialing process prior to practice

PA:

Current licensure as a physician assistant in appropriate state.

Current certification in CPR.

National certification.

Successful completion of Essentia Health credentialing process prior to practice
