Responsibilities and Qualifications

Performs as a navigator in the Ambulatory Telemedicine program Mercy Virtual and in accordance with all applicable Federal, State, and Mercy rules, regulations, policies and procedures, and performs all duties assigned by Medical Director, in the ambulatory program, concerning patients and their care. Facilitate patient/family experience at Mercy virtual by providing coordinated personal, professional and compassionate service. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.


Answer telephone calls from patients/Family in Mercy Virtual Ambulatory ProgramCoordinate Telemedicine visit(s), testing, procedures and/or appointments for the patients in their care team.Facilitate and/or answer patient questions and/or concerns about treatment plan and program.Confirm all patient equipment is delivered and working properly.Educate the patient on all in home equipment and peripherals.Communicate with stakeholders to include patient, physicians, clinics and medical staff.Track and report statistics and information as requested.Schedule all telemedicine visits in the care team in triage software.Participate in the Quality Assurance Program of Mercy Virtual.Participates in the Share governance team.

Knowledge, Skills and Abilities Required:

Education: High school diploma.

Licensure: Class E Driver's License

Experience: Previous experience in a Medical Office or Hospital.

Certification/Registration: BLS required or within 6 months of hire.

Other skills & knowledge: Critical thinking and problem solving skills. Professional verbal and written communication skills with the ability to work in triage software and electronic medical record.

VIEW JOBS 9/13/2018 12:00:00 AM 2018-12-12T00:00 Description This position is a non-licensed/certified staff member authorized to transport and/or stock medications to meet specified essential job duties. This includes staff who do not have the authority to prescribe, dispense and/or administer medications. Under direction of the Director and the Account Sales Manager - perform diversified duties involving contributing to the maintenance of a positive corporate image by providing clients and affiliated company accounts with the highest possible level of service regarding administration of the occupational health program. Assure that proper channels of communication between the account and internal areas of St Luke's are effectively maintained and utilized. 1. Provider Development and Case Referrals and Review (90%): Develop criteria to validate specialist skills within the occupational health and workers' compensation specialty practice. Meet with specialists and obtain commitment on service expectations. Coordinate and communicate care for employees with specialists. Establish and maintain relationships with identified service providers. 2. Review discharge paperwork to ensure compliance to employer and insurance carrier requirements as well as occupational medical practice guidelines. Trend data as appropriate. 3. Prepare written reports regarding injured workers and fax to applicable area client company representative. 4. Provide accounts with courteous and timely responses to account inquiries and correspondence. Ensure that all administrative matters relative to assigned accounts are handled efficiently and in a timely manner. Provide assistance in assuring that proper channels of communication are maintained between the client company, company employees and internal areas of St Luke's. 5. Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety. This tracking may use an employee health database but will require critical review of each patient record to evaluate needs for improvement. In addition, this tracking will be utilized to share the revenue stream to providers. 6. Ensure complete and accurate registration, including patient demographic and current insurance information. Be the system navigator and point of contact for patients, employers, payers and providers allowing direct access for asking questions and raising concerns. Remind patients of scheduled appointments via mail or phone as appropriate. Ensure that referrals are addressed in a timely manner. 7. Coordinate care with injured employees of various area client companies as required. Facilitate processing and treatment of involved employee working through and in cooperation with all designated treating facilities. Place calls to applicable client companies regarding injured employees pertaining to injury. Inclusive of referral need answering any questions as needed. 8. Provide specific medical information to financial services to maximize reimbursement to the hospital and physicians. Review details and expectations about the referral with patients. Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. 9. Considerable knowledge of: medical, legal, or other specialized terminology Other Duties (10%): Protect the confidentiality of information within areas of own position responsibilities by preventing unauthorized release, both verbally and/or in writing. Perform other similar and related duties as required or directed. Observe all health and safety requirements. Must be able to act independently. Transport and/or stock medications Education High school graduate or equivalent Experience 3 - 5 years experience in the occupational health or referral coordination required. LPN or MA preferred. Strong customer service focus. Effective verbal and written communication skills. Teamwork orientation. Organized and able to meet competing priorities. Successful completion of Safe Medication Handling competency. Working Conditions Employee is required to sit continuously and walk occasionally. Employee must lift/carry a light (1 - 20 lbs.) medium (21 - 50 lbs.) load occasionally. Employee is frequently required to bend, squat, lift waist to overhead. Employee is occasionally required to reach forward and reach overhead. For repetitive action, employee is required to use both hands for simple grasping and pushing/pulling. Employee is frequently exposed to electrical hazards and CRT terminal work. Verbal and cognitive, as well as motor coordination and manual dexterity required. Speech, hearing, and near-visual acuity are required. May be required to work weekends and holidays. (Occasionally: 1-33%, Frequently: 34-66%, Continuously: 67-100%) The most significant of duties are included but this does not exclude occasional work assignments not mentioned or developmental duties.
