Job Description

  • Supervises all aspects of the Oncology Navigation Program including Nurse Navigator and Patient Navigators

  • Responsible for weekly time and attendance. Manages both FTP and PTO, and related coverage. Manage staff PTO requests. Identifies staffing needs in advance, assists in utilizing resources effectively. Monitor and control overtime use. Manage weekly reports. Work with Clinical Services team if issue arise.

  • Develops new workflows for navigation efficiency and projects related to navigation.

  • Review weekly Imaging navigation Spreadsheet. Monitors Radiology and Surgeons biopsy numbers.

  • Assist with division of work responsibilities among Nurse Navigators based on volume

  • Reviews Navigation workflows and time management

  • Provides direction for navigators who are not following standard procedures

  • Work with HR to recruit new navigators for open positions.

  • Responsible for Orientation of new Navigators, competency checklists; Sets up orientation plan, Updates Orientation Manuals, Mentor new staff as needed.

  • Involved in Quality Initiative or project, working with a team.

  • Involved in aspects of Preparation for NAPBC survey and NQMBC data submission related to data collection and record review.

  • Provides Annual performance evaluations of Navigation Staff

  • Attends Breast Care Center meetings providing input and suggestions on continuous quality and operational improvement.

  • Attends Weekly Tumor Board Conferences

  • Holds quarterly navigation meetings.

  • Meets with each Navigation team member monthly.

  • Serve as a liaison with Imaging for troubleshooting issues as they arise

Program Statistics Responsibilities:

  • Review and set up new spreadsheets in SharePoint for following year including Accession lists, Monthly Cumulative statistics, Imaging Navigation statistics, outmigration list review, outmigration statistics

  • Review and monitoring of NRC patient satisfaction dashboard. Work with NRC Coordinator to trouble shoot issue.

  • Collect tumor Registry stats quarterly and distribute to designated Supervisor

  • Review Breast Imaging Navigator Data monthly for accuracy.

  • Obtain statistics from the Imaging Department quarterly including but not limited to call back rates, cancer detection rates etc.

Navigation Caseload and Coverage Responsibilities:

  • Manages a limited caseload of patients.

  • Provides coverage for all locations when Navigators are out of the office

  • Navigates oncology patients across the continuum of care beginning with diagnosis through cancer survivorship

  • Adheres to key Touchpoints for Navigation (attached) including introduction to Navigation at the time of cancer discussion, post-operative care planning, follow up care and survivorship.

  • Assesses, plans, and provides patient education and counseling to breast cancer patients providing physical, psychological, and emotional support to patients and their family members.

  • Collaborates with the multi-disciplinary team to ensure continuity of care from one provider to the next. Tracks the patient closely and ensures that care is smoothly coordinated among disciplines using an established tracking system.

  • Completes the Survivorship Plan of care when the patient completes therapy, facilitates discussion with the patient and sends the Plan of Care to the patient.

  • Provides counseling for high-risk patients or those with questions (positive family history, imaging and lab surveillance et al.) and makes referral to medical/surgical team as needed.

  • Communicates with physicians and other members of the interdisciplinary team, within Summit Health, as well as with outside providers.

  • Facilitates scheduling of appointments with other Oncology Departments to ensure timely referral and transition into specialty services.

  • Provides information on maintaining optimal health and prevention of Cancer Recurrence including instruction on the importance of regular visit to their provider for surveillance as recommended by their provider, maintaining proper nutrition and exercise.

  • Documents patient visits and communications in Athena. Updates Accession List and Paper lists. Manages and submits statistical data monthly and as requested to designated Supervisor.

  • Develops a familiarity in chemotherapy protocols to support patients during chemotherapy treatment. Obtains either Chemotherapy/Biotherapy Certification or attends ONS Fundamentals Course.

General Job functions:

  • Sets up community outreach programs and coordinates all fall events with Imaging and the Foundation

  • Manages Oncology Support Groups ensuring there is nursing coverage; Sets up annual calendar.

  • Secures patient education materials and Educational Binders. Orders materials as needed and ensures inventory. Orders give aways for events.

  • Assists designated Supervisor with the development and implementation of a comprehensive Oncology Navigation Program in multiple areas including meeting National Accreditation Standards, involvement with special programs, survivorship, patient education materials, identification of community resources, marketing, developing and prioritizing workflows.

  • Participates in Annual Quality Initiatives as outlined by the Breast Leadership Team.

  • Completes other tasks by designated Supervisor as time permits.

Education, Certification, Computer and Training Requirements:

  • Two Years of Navigation Experience

  • Bachelor's Degree preferred

  • Certification as a Nurse Navigator or Oncology Nursing Society Certification required. Must obtain within one year.

  • Registered RN license.

  • BLS Certification required.

  • Minimum 5-8 years of experience, preferably in Oncology.

  • Ability to communicate in English, both orally and in writing.

  • Ability to use Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voicemail) preferred.

  • Ability to use Standard Office Technology including Microsoft Word and Excel.

  • Experience with an EMR preferred.
