Promotes resolution of social and emotional problems which are or are expected to be an impediment to effective treatment of the patient's medical condition or rate of recovery through direct and indirect services. Provides assistance in dealing with social and emotional problems through evaluation, on-going assessment, counseling and referral to appropriate community agencies.

DOCUMENTATION RESPONSIBILITIES include: completes MSW assessments in a timely manner per agency policy and procedure; documents identified problems, strengths, interventions and progress appropriately in the clinical record; completes recertification and discharge summaries in a timely manner; meets productivity expectations; provides office staff with appropriate reports and documentation in a timely manner per agency policy and procedure; and, turns in weekly schedules and payroll sheets to supervisor according to agency policy.
TEAM RESPONSIBILITIES include: consults with other disciplines regarding clinical work with the patient and caregivers; participates in staff development and training; endeavors to work cohesively with all members of the Home Health team to promote good working relationships; participates in clinical team meetings, utilizes lines of authority appropriately, willingly accepts and completes administrative duties as assigned; provides a positive image of Palmetto Health Home Care and acts as a community liaison between Palmetto Health and the community; and, serves on agency committees as assigned.
DIRECT PATIENT SERVICES include: assesses patients referred for MSW services in a timely manner per agency policy and procedure; develops an appropriate and goal-oriented plan of care in a timely manner per agency policy and procedure; involves the patient and family members in development of the plan of care; informs the patient and family about and facilitates referrals to appropriate community resources; provides appropriate counseling and intervention to patients and families as necessary; observes progress made and alters plan of care based on changing patient needs; reports changes in patient's condition to the case manager and physician as deemed necessary; implements infection control and safety measures per agency policy and procedure; and, provides discharge planning services as indicated.
PROFESSIONAL RESPONSIBLITIES include: pursues professional development opportunities; shares knowledge with other team members through informal and formal means; adheres to agency policies and procedures; participates in the annual performance evaluation by undertaking self-evaluation and establishing personal goals for the following year; and, maintains licensure by fulfilling all requirements.
Education: Master's degree from a school of Social Work accredited by the Council on Social Work Education.
Experience: One (1) year experience as a Social Worker in a health care setting
Number and Type of Team Members Supervised: None.
Licensure, Registry or Certification Required: Licensed Social Worker in the State of South Carolina. Valid driver's license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance. Current BLS (Basic Life Support for Healthcare Providers) required prior to start date directly through the American Red Cross, the American Heart Association, or the military. (To register: Blended Learning or In Person BLS through the American Red Cross; or Classroom or Skills Session BLS through the American Heart Association. Certification must include an in-person skills session.)
Special Training: Ability to effectively communicate with all levels of organization.
Ability to lift 20 pounds and reach heights of five feet. Ability to see, hear and communicate. Ability to perform home visits and assess patient environments of all types.
Driving is required for this position using the employee's personally owned vehicle. Employee must have reliable transportation and will be expected to adhere to the Palmetto Health Driver Safety Policy and specific department driving policies. Employee must pass driver training.
