Job Description:

Performs psychosocial assessments of and on-going direct social work services to patients and families; participates in formulating and implementing discharge plans; identifies unmet psychosocial needs and develops programs to address those needs; and participates in educational activities.

Responsibilities Include:

  • Performs psychosocial assessment of patients and families.

  • Provides introduction and orientation to newly admitted patients/families.

  • Evaluates new admissions by interviewing patients and families, contacting other agencies, if indicated, and reviewing available information.

  • Prepares psychosocial assessments of patients and families using knowledge and understanding of needs of adults/geriatrics.

  • Prepares comprehensive care plans.

  • Participates in interdisciplinary team meetings.

  • Maintains documentation of initial contacts with patient/families and of psychosocial assessments.

  • Formulates and implements psychosocial treatment plans.

  • Employs clinical skills in counseling individuals and families regarding coping/adjusting to illness and placement and discharge planning.

  • Formulates and implements discharge plans.

  • Establishes and maintains professional relationship with patients and families.

  • Incorporates knowledge of community resources and entitlement programs in planning.

  • Negotiates community systems and programs.

  • Collaborates with interdisciplinary team.

  • Implements and continuously evaluates treatment plans.

  • Provides social work services in accordance with required standards.

  • Utilizes knowledge of financial/reimbursement/regulatory systems and requirements in providing services to patients and families

  • Completes MDS forms, as required.

  • Anticipates needs/problems in case management.

  • Maintains documentation of case activity/data.

  • Identifies unmet psychosocial needs and develops programs to address them.

  • Collaborates with staff to identify unmet needs.

  • Participates with staff in group functioning - Patient Council and other patient/family groups.

  • Coordinates family groups.

  • Utilizes knowledge of Social Work treatment modalities in assessing program needs.

  • Represents the facility and department when meeting with community agencies.

  • Performs related duties, as required.

Qualifications

  • Master's Degree in Social Work, required.

  • LMSW required within one (1) year of employment, required.

  • Clinical experience, preferred.

The mission of our Practice Registered Nurse is to provide our patients with a positive experience by delivering nursing care in a professional and courteous manner. In this role, you will be expected to carry out Physician's orders including, but not limited to, assisting with procedures, administering injections, and performing phlebotomy. You will take patient calls, clean and stock exam rooms, call in prescriptions, coordinate supply levels, pre-certify patient's medical services and assist with other day-to-day operations of the office. You must be able to maintain positive communication with patients and team members to ensure a pleasant environment. Responsibilities Include: Chemotherapy/Biotherapy Infusion RN * Performs patient vital signs, ECG and other required tests, record patient history, obtain accurate medication list and any information the physician may request to enable him/her to assist in the diagnosis and treatment of the patient. * Explains normal and abnormal test/lab results to patients and their significant others. * Educates patient regarding medications, invasive and non-invasive procedures. * Serves as a resource to clinical and non-clinical staff. * Communicates with patients, visiting nurses and other physician offices. * Schedules and processes paperwork for patient tests, cardiac catheterizations and other invasive procedures, labs, device procedures or office appointments. Calls in prescriptions. Arranges hospital admissions. * Priorities medical/emergency phone calls; handles according to office protocol. * Ensure exam rooms are clean, stocked appropriately and ready for next patient; ensure equipment is clean/sterile and in working order. * May schedule other clinical staff. Qualifications Graduate from an accredited School of Nursing, required. Bachelor's Degree in Nursing, preferred. Must be enrolled in an accredited program within 24 months of employment, if hired after January 1, 2010 and obtain a BSN Degree within five (5) years of employment date.
