MPCA HealthCorps Program engages AmeriCorps members in individualized outreach attempts using a variety of methods (phone calls, letters, texting, and face-to-face connections (as feasible/appropriate) to connect patients/Medicaid beneficiaries with a primary health care provider at Health Centers. Ensure patients/members receive needed care services, and connect to appropriate supports to access care.

Performs outreach and engagement for at-risk patient populations, including specialized outreach to re-engage patients who have experienced care disruption or barriers to care related to COVID-19. Further help on this page can be found by clicking here.

Member Duties : Member will perform direct outreach to patients/Medicaid Beneficiaries to increase access to health services using motivational engagement, health education, risk assessment, and connecting to supportive resources. Outreach methods include text messaging, phone calls, mail and in-person visits.

Due to Pandemic, face to face and home visits are restricted. Perform risk assessments and address identified social need or barrier to care through resources connections, such as transportation assistance, interpreter support, food assistance, or housing assistance. Coach and motivate patients to engage in primary care for preventive care, chronic condition management, and reduce emergency department overuse.

Coordinate with other members health care team on behalf of patients to communicate needs, provide accommodations, or acquire further clinical support. Provide specialized outreach to re-engage patients who have experienced care disruption or barriers to care related to COVID-19.
