Move In Coordinator - IL, AL, & MC (Tues Thru Sat)

GENERAL SUMMARY:

The MDS Nurse is an RN/LVN who is responsible for the overall supervision, coordination, maintenance and timely completion of the MDS, care plans and medical records for residents.

Essential Job Duties:

1.Facilitate or arrange for supportive educational programs for residents and their families.

2.Coordinates and performs all admission functions within Memory Care, Independent Living, and Assisted Living units. This includes maintaining relationships with referral sources and Piper Shores Community Health Services, participating in pre-admission clinical reviews and the orientation and registration of all new admissions.

3.Provides care coordination and transition planning services for Independent Living and Assisted Living through the Interdisciplinary Team utilization review, and Transition Team process.

4.Provides tours of the facility and pertinent information regarding admissions.

5.Develops relationships and maintains contact with all new admissions for the first 90 days of residency, ensuring that new residents are fully assimilated to their new environment.

6.Orients all new IL, AL, and MC residents to the community, including extensive policy review and sign off and ensuring completion of new resident orientation.
