The NP will independently assess and manage patients in the community setting, identify patient needs, and refer to appropriate resources. The NP will develop a care plan for high risk patients that may involve multidisciplinary care coordination involving pharmacy, social work, registered nurse(s), community health worker(s), and partnering entities such as fire departments. The NP will further support population health/mobile integrated health initiatives including collaboration with local government officials and leading quality improvement activities.
