Job Duties

Hiram W. Davis Medical Center, located in Petersburg, Virginia, is a 94-bed medical facility providing acute medical diagnoses, skilled care, intermediate care, and long term care to individuals receiving services in DBHDS facilities and within the community that have intellectual and/or developmental disabilities. Admissions to HWDMC are generally either permanent placement or special hospitalization. Patients typically come to HWDMC as a special hospitalization for step-down to stabilize, post-surgery rehabilitation, or long-term care.

The Minimum Data Sets (MDS) Coordinator

  • RN completes all required Minimum Data Sets accurately and in a timely manner. The MDS Coordinator collaborates and communicates with interdisciplinary team members to insure timely and accurate completion of their respective MDS sections. The position also provides hands-on support & education to nursing and other departments, such as rehab, SW, dietary, and activities, etc. related to MDS assessment and care planning process.

This position is an hourly wage position and may not work more than 29 hours per week or 1500 hours within fiscal cycle.

Job Duties:

  • Timely & accurate completion of all required Minimum Data Sets.

  • Develops and manages a process for other disciplines in monitoring the timely and accurate completion of MDS.

  • Reviews MDS for completeness, accuracy and facilitates corrections as needed from Case Managers, Social Services, and Nutritionist, Activities, and Occupational Therapy & Physical Therapy Staff.

  • Coordinates & facilitates required and special Team Conferences.

  • Help to identify change of conditions in residents and when change of condition assessments are needed.

  • Monitors appropriate resident bed assignment based on certified level of care.

  • Monitors completion of pre-admission screening and Resident Review (PASRR) on all MI and ID/DD patient admissions to skilled and ICF.

Minimum Qualifications

  • Licensure, or eligible for licensure, as a Registered Nurse with the Virginia Department of Health Professions is required.

  • Knowledge of long-term care resident assessment and care screening along with Federal and State rules and regulations affecting job responsibilities and reimbursement issues.

  • Knowledge and experience in MDS requirements, definitions and completion process;

  • The ability to communicate effectively both orally & in writing.

  • The ability to prepare instructional & training aids.

  • The ability to manage program administrative details.

  • The ability to work independently, manage multiple priorities and experience in resolving problems.

  • The ability to carry out all physical requirements of CPR, TOVA training & lifting, and familiarity with computer applications.

Additional Considerations

  • Experience in long term care resident assessment.

  • Experience with completion of MDS, CAAs, and care plans/ IPOC.

  • Experience with Cerner EHR helpful, but not required.
